Please Note: If you will be hosting a visit to multiple facilities within a system, please indicate employee/bed/procedure numbers seperately for each facility, when possible.

Please identify all areas where your facility can offer opportunities for observation and learning.

These would include 1) areas you manage, and 2) areas outside your supervision where you would be willing to arrange appointments, observation opportunities, etc. for your guest.

Please indicate if your facility offers unique opportunities or if you will be working on or have recently implemented any special projects that may be of particular interest to a program participant.
