    • Copies of utility bills and notices of past due rent or utility bills, eviction/shut down notices, lease agreements
    • Proof of household income, documentation of assistance provided by other agencies, and documentation of other pertinent documents and bills may be required and if requested you will need to submit via email.

  • Please note: For your request to be considered, this application must be filled out in its entirety, and it must include documented proof of financial need/delinquency on company or agency letterhead. In addition, your signature is required to grant Mount Ararat permission to contact any necessary parties to determine the proper appropriation.
