2026 Italy Application, Deposit Agreement, and Deposit Payment

PLEASE NOTE:  AT THE END of this application, you will need to have payment ready to collect your NON-REUNDABLE $100.00 deposit to secure your spot on the team. From there, we will have options to set up your remaining payment for food, transportation and lodging while in Italy.

One Voice Ministry reserves the right to turn any applicant away due to the number of available spots being filled.

We all make mistakes so, to ensure all the information is correct on your plane ticket, we ask that you upload a picture of your picture/information page of your passport below. This is required if you already have a valid passport.

My signature at the bottom of the Personal Commitment section 
registers my own personal commitment while serving with One Voice Ministry.

During the ministry, I will commit myself to:

• Investing in what is necessary on my part to ensure a successful ministry: this includes my willingness to support, encourage, build up, and love my fellow missionaries.
   
• Doing what is necessary to be a part of the team and assure team unity. I realize that complaining to anyone other than the one who can change the situation is inappropriate.
   
• Following the guidelines laid out by One Voice, knowing the parameters are set to ensure a positive experience for everyone.
   
• Realizing that disregarding guidelines will lead to my returning home. Guidelines include: abuse of alcohol, use/abuse of drugs or smoking, continued display of bad attitude, continued display of homesickness, blatant unwillingness to follow leadership, NOT preparing myself physically, spiritually, and mentally. Any expense involved in changing airline ticket, land travel to airport, etc. will be my responsibility.

I do waive and release any claims against One Voice Ministry, their staff and volunteers, and all hosts for injury, loss, damage, accident, delay or expense resulting from the use of any vehicle, any strikes, war, weather, sickness, quarantine, government restrictions or regulations arising from any act of omission of any ship, airline, railroad, bus company, taxi service, hotel, dormitory, restaurant, school, university, or other firm, agency, company, or individual. I also release the Ministries and agree to indemnify them regarding any financial obligations or liabilities that I may personally incur or any damage or injury to personal property of others that I may cause while participating in the Ministry program or programs.
 
I understand that the Ministry is not responsible for, and I release the Ministries from any claims for injury or loss whatever suffered by me during my participation in any aspect of One Voice Ministry. I understand that receiving the appropriate vaccinations is my responsibility and I release the Ministry from any claims for injury or loss whatever suffered by me because of vaccinations I do or do not acquire. 
 
I hereby grant One Voice Ministry full authority to take whatever action they may consider to be warranted under the circumstances regarding my health and safety, and I release them from any liability for such decisions or actions as may be taken in connection therewith. I authorize One Voice Ministry, at their discretion to place me for my welfare at my own (or guardian's) expense and without my further consent and without my guardian's further consent in a hospital within or outside of the United States for medical treatment, or if no hospital is readily available to transport me back to the United States by commercial airline or otherwise, at my own expense or my guardian's expense, for medical treatment. In the event the Ministry or its agents advance or loan any monies to me or incur special expense on my behalf, I and my guardians agree to make repayment within 60 days. 
 
I agree to comply with the rules, standards, and instructions of One Voice Ministry while participating in the various programs. In addition, I also agree to comply with the rules, standards, and instructions for participant’s behavior for the varies programs. I agree to indemnify the Ministry against any consequence of my failure to comply with such rules, standards, and instructions. I agree that the Ministry shall have the right to enforce appropriate standards and that they may at any time terminate my participation in any of the various ministry opportunities and/or programs for failure to maintain these rules, standard, and instructions or for any conduct which the Ministry staff leadership considers to be detrimental to, or incompatible with, the interest, harmony, and welfare of other team members. I also understand that a team leader or the individual or individuals in authority of One Voice Ministry may institute standards and rules in which the program takes place, and I agree to not hold One Voice Ministry responsible for any consequence that may arise because of breaching those rules, standards, or instructions. If my participation is terminated, I consent to be sent home at my own or my guardians' expense, with no refund of fees. 
 
On team outings or activities managed by the Ministries I will accept the authority of the leader of One Voice, whatever matter of choice is presented to the group/team. I will also accept in good faith the instructions and suggestions of One Voice Ministry in all matters relating to the program or the conduct of participants. I understand that from time-to-time publicity materials may include statements by its student’s and/or their photographs and video footage and I consent to such use of my comments and photographs and videos which include me. 
 
I understand that One Voice Ministry reserves the right to cancel, change, or substitute programs in cases of emergency, changed conditions, the interest of each group, insufficient number of participant, or other wise to change initial campus and or adviser assignment, and to make alteration in the program and its activities and itineraries, as may be required in the judgment of the Ministry. In addition, I understand that fees and logistics are based on certain factors over which the Ministry has no control such as changes in currency exchange rates, tariffs, curriculum, inflation of basic costs or strikes, revolutions, wars, and are subject to change. 
 
I understand that the Ministry are relieved of all liability for items lost by the US Mail or and shipping company as well as any airline or other transportation company.  I understand that there are unavoidable risks in travel abroad, and I hereby release the Ministry from any damage, injury (including death) caused by, derived from, or associated with my participation in this program, except for such damages or injury as may be caused by the gross negligence or willful misconduct of One Voice Ministry.

I give the One Voice Ministry full permission and authority to access any and all federal and or state criminal records, and I understand that the Ministry reserves the right to dismiss a participant from any program at any time based on any of the aforementioned records.

I have read the terms and conditions set forth in this agreement and understand they constitute a part of my agreement with One Voice Ministry. I understand and agree to all the terms set forth in this agreement.

I understand that all contributions are made solely for the purpose of holding my position on a mission team with ONE VOICE. Disbursement is at the discretion of ONE VOICE for missions work and evangelism. All team contributions are non-transferable or refundable in the event that the team member does not go or is sent home but will be used to help underwrite the overall mission. I also understand that ONE VOICE is not responsible for transportation, lodging, food, or other inconvenient expenses incurred due to airline delay or flight cancellations while traveling to or from the ministry country. Once accepted, all team members must provide proof of health insurance.
