  • Rental Requests will be processed within 48 hours of receiving this submission.

    To ensure availability, it is highly recommended to complete this form at least one week before the rental is needed.

  • Will this driver operate within a 150 air-mile radius from their normal work reporting location?*
  • Will this driver return to their normal work reporting location at the end of each day?*
  • Will this driver drive between 14-16 hours in any 24 hour period?*
  • Will this driver drive more than 11 hours in one day or work more than 14 hours in one day?*

  • This driver is not ELD Exempt and will require a ELD be on board the vehicle along with the vehcile folder with all required documents.

    This Rental request will also be sent to HQ to notify the appropriate warehouse for fulfillment.
  • Will the rental be for 8 days or fewer?*

  • This rental is an 8 days or Fewer. You will need to print a copy of the 8 Days of Fewer ELD Exemption paperwork that will be attached to the email copy of this request.

    *This paperwork has been extended through 10.12.27. Having the expired paperwork is acceptable.*

  • This driver is Short Haul Exempt and will not require an ELD or any paper logs to be on board the vehicle.

    You will recieve a Driver Statement of On-Duty Hours and a Short Haul Driver Breakdown attached to the email copy of this request.

  • All drivers are to be approved prior to driving a CMV, followed by completing the required driving skills test. In additon, the manager requesting will be responsible to confirm with the Transportation Team on ELD requirements.
