If you are getting a tattoo 6 inches or larger, please attach a photograph of the area of the body we’ll be working on (For full body tattooing, please wear underwear or a bathing suit). Please make sure to take your photo in a well lit area from at least 2 feet away. If possible, have a friend take your photo or use your phones selfie timer. Please do not send images taken in a mirror.

I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up via email within 48 hours, and will be required to send a deposit via PayPal or Venmo within 24 hours of agreeing to an appointment time with the artist. I acknowledge that until the deposit has been received my appointment has not been confirmed. I understand that this is a non refundable deposit. If I need to cancel or reschedule the appointment I will do this no later than 48 hours before my appointment to avoid losing my deposit. I understand that if I make changes to my design after submitting this request and booking the appointment the cost may vary from the quote I was given.

Please know that LHCTX has a ZERO TOLERANCE POLICY pertaining to racism, sexism, homophobia, transphobia and violence of any kind. If you display any of these traits during your appointment your session will be terminated and you will be asked to leave the property IMMEDIATELY!
