permission to attend the Camp Shamineau retreat.

In case of emergency, I give permission for the chaperones or camp staff to take the actions needed to insure the safety/health of my child. If a medical need does arise, chaperones will contact parents/guardians. I relieve the school of liability due to injury/accident. If my child needs to return home before the trip is over, I will assume responsibility for transportation.

Students: I pledge that I will cooperate fully with the trip advisors/chaperones. I will follow the school rules and instructions given to me. I accept responsibility for my behavior asa representative of Legacy Christian Academy and pledge to conduct myself in a manner that will reflect positively on my school, family, and our Lord Jesus Christ. Upon violation of school lifestyle policies or the school handbook, dismissal from the trip will be considered by the group leader in consultation with the school administrator. My Parent/Guardian will be notified to pick me up if a violation occurs.

Rooming Arrangements: While we cannot guarantee that students will room with everyone they would like to, we do work hard to ensure at least one or two requested students room together. Please list up to three students your son or daughter would like to be roommates with while at Camp Shamineau.
