AUTHORIZATION FOR TRANSPORT – SALSI CAMP – ASD SALSI ACADEMY
The undersigned parent/legal guardian of the minor enrolled in the Salsi Camp and participating in the SALSI CAMP week organized by ASD Salsi Academy hereby authorizes ASD Salsi Academy, with registered office at Via Ligabue 42, 42020 Quattro Castella (RE), to transport the minor to and from the gym and the village for all weeks of activities organized by ASD Salsi Academy, using vehicles driven exclusively by members of the Salsi Camp staff.
This consent is valid for all weeks of activities, without the need to specify additional dates.
DECLARATIONS AND CONSENT
The parent/legal guardian declares that they have been informed about the transport arrangements, the vehicles used, and the fact that they will be driven exclusively by members of the Salsi Camp staff.
ASD Salsi Academy and its representatives shall not be held responsible for any damage or accidents resulting from non-compliant or negligent behavior of the minor during transport. Any damage caused by the minor to persons, vehicles, or property shall be compensated by the parent/legal guardian.
This consent is valid for all scheduled weeks of activity and does not require further authorizations.
Transport will be organized according to vehicle and seat availability.
The personal data of the minor and of the parent/legal guardian will be processed by ASD Salsi Academy in compliance with EU Regulation 2016/679 (GDPR), exclusively for purposes related to the organization of transport and sports activities.
The parent/legal guardian also declares that they have read and understood all the above conditions and provide their free and informed consent to the transport and to the minor’s participation in the activities.
