Step 2: Collection

Complete the following steps for proper collection.
1. DATE OF BIRTH ON LABEL: Using a smudge-free pen, write your date of birth on one of the tube labels and place it lengthwise on the tube.*

2. REFER TO THE INSTRUCTION GUIDE IN YOUR KIT: Use the included instructions to complete your blood collection. Follow all steps carefully. Please fill the tube with blood to the '600' mark (top line).
The following video may be helpful.
3. PROVIDE COLLECTION DATE & TIME: Please provide the date and time you collected your blood sample. You may also write this information on the label attached to the biohazard bag.

Step 3: Return

It's now time to prepare your sample for return. Please ensure you have completed the following steps.
1. PLACE TUBE IN BIOHAZARD BAG: Ensure the cap is fully snapped onto the tube and place it in the biohazard bag.*

2. PLACE SAMPLE IN RETURN BOX: Dispose of all materials that came with the kit, then place your sample in the return box with the label. Seal the box using the adhesive strip.*

3. DROP OFF AT FEDEX INDOOR LOCATION: Important: You must collect and ship your sample on the same day.
