Bishop Dwenger High School - Indiana School Choice Scholarship Information Form 2026-2027
With the passing of Universal School Choice by the state legislature, removing income limits, the 2026-2027 Indiana School Choice Program will require each family to provide proof of residency in order to receive the Choice Scholarship for the school district in which they reside. In joint custody situations, please provide proof of residency for the parent who has primary physical custody (50% or more).

Parents: Please complete one form per family for your students attending Bishop Dwenger High School and upload your proof of residency. Options for proof of residency include:
- 2025 Federal Tax Return (page 1 only)
- Drivers' license or state issued ID
- Bank Statement or Utility Bill within the last 90 days
- Current lease agreement/mortgage statement
- Current Pay stub

Once documentation has been submitted and processed, an endorsement form will be sent to the email address you provided above. The endorsement form directs your student's School Choice award amount to Bishop Dwenger High School.
