Puppy Application at AZ Cavapoos we take great pride in raising healthy & happy puppies. We raise them in our home and they are all trained with ENS, ESI, potty training and crate training. All of our puppies come with a 2 year health guarantee, they will start potty training at 3 weeks, vaccinations and de-wormings will all be up to date, and puppy will be microchipped. When you pick up your puppy they will come home with a puppy go home pack. This pack includes health guarantee ,shots, and vet clearance, among other treats for your new family member

I First Name Last Name understand that my waitlist fee of $200 is nonrefundable and is put towards the price of chosen puppy. Once said puppy is chosen the puppy deposit is due and the remaining balance is due at pick up or if using a flight nanny 1 week prior to delivery.
