This Low Cost Spay/Neuter Clinic on 04/30/25 provides $50 spay and neuter surgeries for selected cats and dogs in DeSoto County Mississippi. 

PROOF OF INCOME OR GOVERNMENT ASSISTANCE MAY BE REQUESTED IF YOUR APPLICATION IS APPROVED.  NO NEED TO SEND ANY PROOF WITH THIS APPLICATION. 

The Low Cost Spay/Neuter Clinic charges $50 per dog or cat, with a maximum of two pets per household. Each pet requires a separate application. Space is limited, so act quickly!

Since an up-to-date rabies vaccination is legally required, there will be an additional fee of $20 per animal unless you provide valid rabies vaccination documentation from your veterinarian’s office via email upon approval. Please note that a rabies tag is not acceptable as documentation.

 

Dogs and cats must be 12 weeks old for surgery. 

 

Terms -

I, the applicant, agree and acknowledge that:

I am submitting an application for a spot in the clinic occurring on Wednesday, April 30th; however, I understand and acknowledge that such spot is not guaranteed despite my application being received.

I understand that clinic space is limited, my application is subject to review, and that if I am selected to partake in the clinic, I will be contacted by PetWorx on or before Monday, April 14th and payment in full will be due by Thursday, April 17th, 2025. 

I verify that I am the owner to the pet listed on the form for spay/neuter.

I verify that I am a current resident of DeSoto County MS, and will prove such residency if requested.

I will send any proof of benefits/income that I have stated on this form and a picture of my government issued ID to be considered.

I understand that PetWorx is offering a discounted spay/neuter and that any other fees incurred will be my responsibility (such as rabies shots, flea/tick treatment, and any other treatment or procedure other than the spay or neutering services as applicable). 

I understand that PetWorx, The City of Hernando MS, Hernando Mississippi Animal Shelter, PetWorx Sponsors, Clinic Sponsors, and any contracted veterinary staff are in no way responsible for the health of my animal or anything related to the spay/neuter surgery. I will review the release of liability below to confirm my understanding of the foregoing. 

I understand that I, the pet owner, am solely responsible for all medical care for my pet, both before and after the spay/neuter surgery, even if as a result of the spay or neutering services.

I understand that I am responsible for the scheduling and transport of my animal to the clinic, and I will bring my pet in a crate (if a cat) and on a leash or in a crate (if dog). Such pets are to remain in a crate or leashed and in my control until instructed otherwise by clinic staff or volunteers.

 I understand that I am required to be ON TIME to drop off and pick up my pet. If I am more than 20 minutes late for my scheduled pick-up time, there will be an additional $40 penalty fee paid in cash and independent of the cost of the spay or neutering services when I do arrive.

If I fail to pick up my pet, PetWorx will be forced to consider this as abandonment that could result in PetWorx contacting the proper authorities to pursue charges.

If approved, I understand that I am responsible for making sure that my pet DOES NOT EAT ANYTHING after midnight on the night prior to surgery. I understand and agree that, if I do not prevent my pet from eating the night prior, this will greatly increase the risk of complications during the procedure that could result in the death of my pet and will not pursue action against PetWorx, The City of Hernando MS, Hernando MS Animal Shelter, any PetWorx sponsors and any contracted veterinary staff as a result. 

I understand that communication is mainly via email for this application. I will also check my spam mail to ensure I have not missed any important and time sensitive correspondence. 
 
 Each of PetWorx, The City of Hernando, the Hernando Mississippi Animal Shelter, and any contracted veterinary staff has the absolute right to refuse service to anyone at any time.

Each of PetWorx, The City of Hernando, Hernando Mississippi Animal Shelter, and any contracted veterinary staff has the absolute right to refuse service to any animal that is deemed a health risk.  

 

 






 


 

 

Liability Waiver-

I understand that the operation presents some hazards and that injury or death may be a result, for there is some risk in the procedure and the use of anesthetics and drugs in providing this service. 

I understand that some factors significantly increase surgical risks, including but not limited to: Pregnancy, heat, and disease such as FIV, FLV, and Heartworms. 

I understand that serious complications can occur as a result of this surgery, including but not limited to: bleeding, nerve damage, bowel or urinary problems , blood clots, heart attack, allergic or other reactions to drugs or anesthetics, pneumonia, inflammation, and adhesions.

I understand that individual pets may respond differently to anesthesia and because of the stress of a surgical procedure, unexpected reactions may occur. These include allergic reactions, abnormal sensitivities, and the absence of regulatory mechanisms in some pets. Such complications are unpredictable, and in rare cases, may cause death.

I understand that my animal will not receive pre-operative blood work at this clinic. I understand that if this is important to me as the pet owner, I will need to arrange with a veterinarian to have blood work drawn at my own expense prior to the date of surgery.

 I have been informed and understand the risks associated with any type of surgery and that in some cases , my pet could die or suffer serious effects during or after surgery. Acknowledging and understanding the full extent of these risks and dangers associated with surgery, I hereby release and discharge PetWorx, The City of Hernando Mississippi, The Hernando Mississippi Animal Shelter, PetWorx Sponsor Companies, Clinic Sponsors, and any contracted veterinary staff (Themselves, Officers, Officials, Employees, agents, representatives, directors, volunteers, contractors, affiliated entities) from any and all actions, claims, damages, loss of service, expenses and compensation on account of the surgical services to be performed and authorized by this form or otherwise arising out of my attendance to the clinic.

Applicants who are approved will be notified by Monday, April 14th. Full payment, along with rabies documentation (if opting out of the rabies shot), must be submitted by Thursday, April 17th to secure your spot. This payment is non-refundable.

Important: Approved applicants must arrive on time for the clinic date. Late arrivals will lose their pet’s spot, along with their payment.
