Injury Liability Waiver 

I hereby acknowledge that I am voluntarily participating in sports activities organized by the Kicks 4 Growth. I understand that these activities involve inherent risks, including but not limited to bodily injury, property damage, and death, which may occur as a result of my participation. Therefore, I agree to the following terms:

RELEASE AND WAIVER OF LIABILITY: I hereby release and forever discharge the Kicks 4 Growth, its directors, officers, agents, employees, and volunteers (collectively, the "Released Parties"), from any and all liability, claims, demands, and causes of action that I may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise.

ASSUMPTION OF RISK: I understand and acknowledge that my participation in the sports activities organized by the Kicks 4 Growth involves inherent risks, including but not limited to bodily injury, property damage, and death. I voluntarily assume all risks associated with my participation in these activities.

INDEMNIFICATION: I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, demands, and causes of action, including attorneys' fees and costs, arising out of or related to my participation in the sports activities organized by the Kicks 4 Growth.

MEDICAL TREATMENT: I authorize the Kicks 4 Growth, its agents, employees, and volunteers, to obtain or provide medical treatment for my child in the event of any injury, illness, or other medical condition that may occur during my participation in the sports activities.

ACKNOWLEDGEMENT OF UNDERSTANDING: I have read this sports waiver, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue the Released Parties for any injury, death, or damages that may result from my participation in the sports activities organized by Kicks 4 Growth.

Photo and Video Release 

Ihereby grant and authorize on behalf of the following minor the right to take, edit, alter, copy, exhibit, publish, distribute, and make use of any and all pictures or video taken of Minor(s) to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits, and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. 

This authorization extends to all languages, media, formats, and markets now known or later discovered.

This authorization shall continue indefinitely, unless I otherwise revoke this authorization in writing, or the Minor(s) revokes this authorization in writing upon reaching majority age.

I waive any right that I or the Minor(s) may have to inspect or approve any finished product in which Minor’s likeness appears, including written or electronic copy.

I agree that Minor(s) has/have been compensated for this use of Minor(s) likeness or has otherwise agreed to this release without being compensated. I waive any right to royalties or other compensation arising or related to the use of the photograph. 

I understand and agree that these materials shall become the property of  Kicks 4 Growth.

I hereby hold harmless and release all liability, petitions, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate and Minor(s) estate(s).

I warrant that I am of full age and have every right to contract for Minor(s) in this regard. I state further that I have read the above authorization, release, and agreement, prior to its execution, and that I am fully familiar with its contents. 

This release shall be binding upon Minor(s) and me, and our respective heirs, legal representatives, and assigns.
