To place an order, you may use CashApp to send funds to our CashApp business account (see instructions below), or you may order during an appointment, which can be scheduled by clicking HERE. 

^ These are the only two options at this point. 

For compounded tirzepatide, I recommend ordering during an appointment only to ensure it is currently available.

Instructions for CashApp: 

If you do not have a CashApp account currently, click HERE to sign up and you will also receive a $5 credit for signing up. CashApp is an application that is downloaded to your smartphone.

- If you already have a CashApp account or are able to sign up, follow these instructions:

1. Open the app
2. Navigate to the “$” at the bottom of the screen
3. Type the amount you would like to pay
4. Select “Pay” at the bottom
5. On the screen that then displays, you will type “$SmartHealthWellness” in the “To:” box and you will see my picture on the profile confirming it is me.
6. In the “For: Add a note” section, type how many units you are trying to order and either “office pickup” or “ship to [insert your shipping address]”. If you would like shipping to home, make sure you include $40 extra in your order to cover this!

200 units: $160
450 units: $340
600 units: $420 (this comes as three 200 unit vials)
900 units: $625 (this comes as two 450 unit vials)
—> The pharmacy does not offer any other vial sizes so please only send payment for these sizes or I will need to contact you to correct your order!

Example: If I wanted to order 450 units and I want it shipped to my house, I would send $380 and in the note section I would put “450 units, ship to [my desired shipping address here]”.
- If I was ordering this same vial size for office pickup, I would send $340 and in the notes would put “450 units, ship to office”.

Please schedule an appointment HERE to order men's health medications. You may only order during an appointment. The appointment can be via video IF you have been seen in person within the last year. 

If you have not had labs within the last 3 months, you have 2 options:

1. Schedule your appointment to order in the morning preferably before 10am and come fasting (nothing to eat or drink except water for 12 hours) so that we can collect blood at the time of your appointment. 
2. You can order a lab kit from Rupa to take to the OSF Polyclinic lab (707 N Logan Ave, Danville, IL) any time Monday - Friday 7am - 5pm. The Polyclinic labs allows walk-ins so no appointment is required for this. They do charge $35 for the draw and this is in addition to the cost of the lab kit. If you would like this option, click HERE, add the "Smart Testosterone Follow Up" kit to your cart, check out, wait a few days for the kit to arrive to you in the mail, and then take it to the Polyclinic Lab. You should be fasting and this should ideally be before 10am for the most accurate results.
