By signing this document, I have ensured that my pet is currently up to date on their Rabies vaccination, all information entered is correct, and I am giving consent for the Friends of Manchester Animal Shelter to keep this form on record. I am aware that bringing my pet to the shelter enviornment can be potentially stressful and an uncomfortable situation for my pet. I agree that I will keep my dog on a leash, my cat in a cat carrier or my small animal in a small animal carrier until it is time to take the photo. I understand that everything in process is for the safety & benefit for myself and my pet. 

I also understand that I am not allowed to bring my animal into the shelter for any reason. Although there are Open Hours also occuring at this time, I understand that this does not mean I can bring my dog/cat/small animal through the front door.
