      • Member authorizes San Antonio Prime Wellness (SAPW) to charge monthly membership fees to the credit/debit card provided.  If the member’s credit/debit card has changed, the member must notify San Antonio Prime Wellness as soon as possible.

      • Members are responsible for attending their appointments or communicating with SAPW of any adjustments in their schedule.  

      • Memberships are non-transferable and may not be shared; Any membership payments are non-refundable.

      • SAPW reserves the right to change clinic policies, regulations, and pricing at any time upon providing reasonable notice.

      • The member acknowledges this document as an agreement and will become legally binding upon its acceptance by SAPW.

      • The member acknowledges receipt of SAPW membership terms and conditions and has read, understands, and agrees to be bound by the terms and conditions as part of this agreement.

      • The member assumes full responsibility for services received at SAPW and shall indemnify SAPW, its affiliates, agents, and employees against any and all liability arising from services rendered. 

    • Direct Patient Care Membership Terms and Conditions

      What is Direct Patient Care?  Sixty percent of patients wait two weeks for an appointment with their primary care physician, and only 10 percent can see their provider on the same day.  Direct patient care is an innovative way to bring access to providers in a low cost and time efficient manner.  It makes high-quality primary care accessible for the insured, uninsured, and under-insured (high deductible) members of our community.  It is a system that eliminates third-party insurance costs so providers can focus more time and energy on patient relationships and outcomes. Patients see familiar providers for all visits which are usually scheduled for 30 minutes.

      • Patient’s must apply for membership in our model. The application consists of a signed HIPAA Authorization, a completely filled in General Patient Information and Medical History Questionnaire, and a Medical Symptoms Questionnaire. This is to ensure SAPW keeps a small patient panel to allow better access to providers. Because of smaller patient panels, SAPW is able to offer same or next-day appointments with minimal waiting.  
      • Once the application is reviewed and approved, the cost of Membership is a $110 per month. The membership includes:

      • A blood draw for laboratory analysis of metabolic health
      • Return visit for a Complete Physical exam and review of laboratory data with a provider (60 minutes)
      • Up to two 30-minute visits (office or tele-health) per month for duration of membership after one in-person physical exam per year.  The unused visits of any month will not carry forward.
      • Additional 30-minute office or tele-health visits are $50
      • There may be unforeseen circumstances when a member needs to terminate their membership.  SAPW requires a 3-month cancellation notice.

      • Covered Services of the DPC Membership include wellness exams and management of acute and chronic diseases
      • One page work excuses are included. Forms provided by patient that need to be completed by SAPW that are more than one page (for example, but not limited to, disability, FMLA, attorney correspondence) will be charged $10 per page.

      • Anything not specifically listed as a Covered Service shall be a non-covered service. Any health care services not performed on or within the premises of SAPW including emergency room visits, hospital stays, specialist care, imaging and labs, medical equipment (braces, splints, etc.), and any care delivered by providers not affiliated with SAPW is a non-covered service
      • Patients will utilize the “Book Consultation” button on SAPW’s website https://sanantonioprimewellness.com/ as the primary way to request an appointment.  You must state your name, phone number, email, and the reason for the appointment. This will efficiently communicate to SAPW via email of your request for an office or tele-health consultation.  Patients need to call 911 for urgent or emergency situations

      • Office or tele-health visits are same day or next day appointments (up to 30 minutes) and occur only during regular business hours, excluding holidays
      • Direct Patient Care IS NOT a CONCIERGE practice model. We operate within normal business hours and do not do home visits, hospital, or other in-patient setting evaluations

      • By entering into this membership agreement, you acknowledge that SAPW does not provide health insurance coverage and that this is not a contract for insurance.  SAPW provides only the Health Care Services specifically described herein and additional costs for laboratory (or any laboratory costs not covered by patient’s insurance), medical imaging, surgery, specialist care, emergency department visits, and hospitalization required outside SAPW’s services are the responsibility of the member and should be paid to the pharmacy, laboratory service, and/or imaging center at the time of service.  SAPW encourages you to combine SAPW Direct Patient Care membership with appropriate health insurance coverage.  If you are insured with an HMO, you still need to have a primary care provider. Since we DO NOT contract with insurances, we cannot be your primary care provider. We cannot authorize or generate referrals on behalf of HMO patients.
      • SAPW has elected to opt out of Medicare. That means none of the services we offer can be submitted to Medicare for reimbursement

      • Diagnostic lab test fees (if not covered by insurance) are available at significant discount to members
      • SAPW will explore alternative pain management options rather than prescribing narcotics as part of the patient’s care

      • All Fees paid are non-refundable. This includes all Fees that may have been paid whether such were paid on a monthly or annual basis
      • Services will not be rendered for patients with past due accounts

      • Patient understands and agrees that SAPW offers communication via email for matters such as lab results, scheduling appointments, and communication regarding filled prescriptions.  Although SAPW has implemented reasonable technical safeguards, SAPW cannot guarantee privacy, security, or confidentiality of emails sent or received.  SAPW is not responsible for emails that are lost due to technical failure during composition, transmission, or storage.  SAPW will not forward emails to third parties without your prior written consent, except as authorized or required by law.  Patient understands and agrees that email communications are not encrypted.  SAPW respects and protects the privacy of our patients.  SAPW will never sell or rent email addresses to third parties.  Patient may discontinue receiving emails as a means of communication by sending an email or letter to SAPW. 
      • To cancel an appointment, patient must call 210-455-3309 at least 24 hours prior to the scheduled appointment or, regrettably, a $100 no-show fee will be charged.
