  • 2025 Arts Grant Program Final Report

  • Pursuant to terms detailed in the City of West Hollywood Grant Agreement, the GRANTEE shall submit a Final Grant Report Form electronically within 30 days of completion of the project.  The form will describe the use of the grant, marketing and public outreach efforts in association with the grant project, and a final itemized report of actual expenditures and revenues. This information will be used, in part, to develop quantitative and qualitative outcomes for the City of West Hollywood's Arts Division programs.

    To facilitate this reporting requirement, the form that follows should be used to submit the report electronically via this platform.  Please complete Part I: Organization information and click on the Save/Submit Button.  Your final report will be registered and you will receive an email as indicated below with the {edit_link}.
