The order confirmation form protects you (Customer) and us (Lizo Luxe Hair) from any fraud related purchases. This form is an authorization of purchase from you (Customer) to Lizo Luxe Hair. You must complete this entire form after purchase in order for us (Lizo Luxe Hair) to process and ship your order. If the order confirmation form is not filled out to its entirety within 2 business days from purchase, you will be refunded and subjected to a $200 restocking fee. Also, if you have purchased using one of our installment options like Klarna or AfterPay, there will be a 15% restocking fee plus the $200 restocking fee.
