By submitting this Application form, you consent to the photo release, the terms and conditions of the Kampala Baptist Church - Children's Ministry as a volunteer as clearly indicated bellow.
I grant permission to KAMPALA BAPTIST CHURCH "Children's Ministry", and its agents the irrevocable and unrestricted right to produce photographs and videos taken of myself while at KAMPALA BAPTIST CHURCH or as i execute my voluteer dutes under the "KBC Children's Ministry" for any lawful purpose including publication, promotion, illustration, advertising or historical archive in any manner or in any medium by the KBC Children's Ministry or Kampala Baptist Church.

I hereby release the KBC Children's Ministry and Kampala Baptist Church and its legal representatives from liability for any violation or claims relating to said images or video.

Furthermore, I grant permission to use the statements of myself given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area.

I waive my right to any and all compensation stemming from the use of these materials.
