    Hi! My names Abbey! Thank you for coming to fill out a submission form! Very excited to hear about your ideas & can't wait to work with you!
  • The purpose of this form is to get a clear idea of what you're looking to have tattooed! Please read & answer questions carefully!

    This form can also be used to book a consultation.

    Once I receive your submission, I will review it and reach out to you for booking & furthur details if needed :)

    Some important information before you begin!

     

    ✧ My books are open 2-3 months at a time! 

    ✧ My minimum is $120. My hourly rate is $170. (+HST)

    ✧  A non-refunable deposit of $100.00-$150.00 will be required to book an appointment, information for paying deposits will be sent during the booking process! Please do not send any money until you are asked!

    ✧  You can submit multiple ideas in one form, just please keep them as organized as possible!

    ✧ I do not send out designs before hand, all designs are shown at the appointment, so please be as descriptive as possible in this form! Small changes can be made the day of the appointment.

    ✧  Please allow up to one week to hear back from me as I am only one human! Thank you for your patience!

     

    Reminders:

    I will not tattoo anyone under the age of 18. Even with parental consent.

    I am currently focusing on black and grey work, and will not be taking on any full colour pieces! I do enjoy pops of colour or small colour pieces!

    You cannot get a tattoo while pregnant or breast feeding.

     

    Policies:

    Deposits

    The deposit works as a placeholder for your appointment date! Your appointment is not confirmed until the deposit is recieved and should be sent within 24hrs to secure your appointment.

    Each deposit allows for ONE reschedule, anything after that will require a new deposit to secure a new appointment. The deposit is good for one year, if another appointment isnt booked within that time frame the deposit is forfeit.

    If you arrive more than 20 mins late to your appointment this may result in the forfeit of your deposit and your appointment being cancelled.

    Please note that if you change your concept your deposit is also forfeit and a new deposit will be required to book. 

    Deposits are non-refundable, so please be confident that you want to book the tattoo before sending your deposit.

     

    Rescheduling:

    A minimum of 72 hours notice is required to reschedule, a deposit is good for one reschedule and if you choose to cancel your deposit will be forfeit.

    Please let me know ASAP if you cannot make it to your appointment so we can get your rescheduled without impacting your deposit.

    *there are exceptions to the 72hrs notice: snow storms, road closures, etc.*

    Notice to cancel or reschedule that is given on the same day as your appointment will be treated as a no show.

    No Show Fee: $250 (in addition to losing your deposit)

    Please just give us ample notice as I really don't want to have to charge you this fee. If we are not given enough time to fill the slot, we will be forced to enforce this policy. Please remember this is our only source of income, if we're not tattooing, we're not earning anything.
