  • Once we have received your information and your payment, we will contact you to set the appointment.

  • The consultation includes a detailed preparation by ELZA’s specialists with the data that I have provided and a 20-minute online consultation. My data will be deleted if there is no further treatment at the ELZA Institute. I am aware that this represents an evaluation of medical data and not a final ophthalmological assessment. There is no entitlement to treatment.

    I understand that my payment is a deposit for the in-person consultation. The final costs may be higher, depending on additional examinations that might be required. In such case I agree to pay these additional costs on-site (credit card, cash).
