Once your application has been submitted, you will receive a confirmation e-mail. A copy of the confirmation will also be sent to the director. You also need to print and complete the signature page which can be found at www.ohband.org. Then secure the necessary signatures and return to your director with the application fee for each instrument on which you are registered. If you are unable to print the form, your director can supply one for you. You only need to complete ONE signature form listing all instruments on which you are auditioning. Your director must scan and email all completed signature forms to sschoch@wtps.org. Application fees must also be mail and postmarked by the registration deadline posted at the top of this form. If you have questions or problems with this form, please contact Shannon Schoch at sschoch@wtps.org If you need to make corrections or changes to the information you have submitted your form, you must complete a new form and your original form will be deleted. We will keep the most recent submission and delete all previous submissions. AFTER HITTING SUBMIT, PLEASE ALLOW UP TO 10 MINUTES FOR THE SYSTEM TO PROCESS YOUR EMAIL CONFIRMATION.
