CONFIRMATION AND SIGNATURES

By signing, I, as an adult over 18 years of age, or as the Parent/Legal Guardian of applicant, hereby agree and grant permission for The Spaatz Association to use comments, quotes, or photographs of me, my image, or my likeness, as a grant recipient, for any lawful purpose, including brochures, illustrations, marketing, or for web content to promote the Association and its services.

I understand that funds are distributed in advance and are expected to be used in pursuit of education as listed in my application. If I change my education plan, I will contact The Spaatz Association immediately at ScholarshipsAndGrants@Spaatz.org for determination if unspent funds are to be returned or if I may use them for my new plan.

If awarded a Spaatz Association Academic Scholarship (Post-Secondary), if requested, I agree to provide the Association with documentation of appropriate use of scholarship funds, such as receipts, photos, completion certificates, or narratives of use within 90 days of expenditure of funds via this form (https://form.jotform.com/262735683305057).

I acknowledge that I must return my scholarship to the Spaatz Association if it is not used within 12 months of the award date. Failure to comply with these requirements will make me ineligible for future Spaatz Association grants and scholarships.

I, the undersigned, give to The Spaatz Association, Inc. (the “Association”), and to anyone authorized by the Association, my irrevocable and unconditional consent to use and reproduce a photograph containing an image of myself for use in any media or medium, including the Association’s Web site, for the purpose of promotion, marketing and publicity in connection with the Association or the Association’s grant program. Such photograph may be used and reproduced by the Association at any time and in any manner without compensation to me, and I waive any right to inspect or approve any such use. A description of the photograph(s) is as follows: All photos, audio, video, articles and other media related to the event that I provide.

I also agree that the Association may use my name and biographical information to identify me as the subject in the photograph in connection with the uses specified above.

I also agree that if I hold any copyright in the photograph, the Association is hereby granted a perpetual license in the copyright to the photograph in connection with the uses specified above.

I acknowledge that I am responsible for gaining permission from anyone depicted in the media provided to The Spaatz Association to use the photographs as stated in this consent. My submission of media to The Spaatz Association is my affirmation that I have gained permission from those persons.

I understand that the Association is relying on the consent given above and I agree that I will not at any time withdraw the consent I have given.
