Pre‑Approved ACH Authorization for Per Capita Processing
Purpose
This Pre‑Approved ACH Authorization allows the Wisconsin State Lodge, Fraternal Order of Police (“State Lodge”) to withdraw funds from the designated bank account of the Local Lodge listed below. These withdrawals are used exclusively to process State and National per capita charges for new members added into the FOP member management system and for annual per capita billing.
Authorization for New Members
By signing this authorization, the Local Lodge grants the State Lodge permission to initiate ACH withdrawals upon the entry of new members into the FOP member management system. The entry of a new or reinstated member into the system by the Local Lodge shall constitute authorization for the State Lodge to process the required State and National per capita needed to bring that member fully current with the Local Lodge’s membership cycle.
This authorization covers:
- New members entered into the FOP member management system
- Reinstated members entered into the FOP member management system
- Automatic processing of State and National per capita based on the half‑year schedule (January–June / July–December)
Annual Per Capita Billing
Annual per capita will be charged at rates set by the Wisconsin State Lodge and the National Fraternal Order of Police.
The billing schedule is as follows:
First Half Per Capita
- Covers January 1 – June 30
- Due November 1
- Includes a $25 annual administrative fee required by the National Lodge
- Local lodges must have all membership updates completed in the FOP member management system no later than October 15
Last Half Per Capita
- Covers July 1 – December 31
- Due May 1
- Local lodges must have all membership updates completed in the FOP member management system no later than April 15
After each due date, the State Lodge will withdraw State and National per capita based on the number of active members listed in the FOP member management system. Local lodges must ensure all membership updates are completed by the required date.
Members are considered active for the half‑year period for which per capita is paid. If a member will not be renewed for the upcoming period, they must be set inactive in the FOP member management system before the update‑by date.
Local Lodge Responsibilities
The Local Lodge agrees to:
- Maintain an active ACH authorization on file with the State Lodge
- Understand that entering a member into the FOP member management system constitutes authorization for per capita withdrawal
- Ensure sufficient funds are available at the time of withdrawal
- Complete all membership updates in the FOP member management system by October 15 and April 15 each year
- Notify the State Lodge immediately of any changes to banking information
- Keep local records of membership end dates when members are removed from the system
State Lodge Responsibilities
The State Lodge agrees to:
- Withdraw only the amount necessary to bring each member current
- Process ACH transfers securely and maintain accurate records
- Provide confirmation of each withdrawal
- Notify the Local Lodge of any issues preventing processing
Duration
This authorization remains in effect until revoked in writing by the Local Lodge. Revocation does not cancel or refund any per capita already owed or processed.
