Adult Recess Event Liability & Model Release Form

I, the undersigned, understand that by signing up for Adult Recess, I am willingly subjecting myself to activities that may cause sweat, laughter, and maybe a bruise or two (it’s dodgeball, after all). I acknowledge that these activities—designed for fun, not finesse—carry inherent risks, including but not limited to:

• Minor scrapes, bruises, or an unexpected dodgeball to the face.
• Muscle soreness from overestimating my athletic ability.
• Embarrassment caused by tripping over my own feet or dropping a ball that was passed perfectly to me.

I fully understand these risks and take responsibility for my coordination (or lack thereof). I willingly participate knowing that Adult Recess may be equal parts joy and chaos.

In exchange for the privilege of reliving my playground days, I hereby release and hold harmless Adult Recess, its organizers, volunteers, referees, and anyone else who may witness my less-than-graceful moments from any and all liability. This includes (but is not limited to) claims of:

• Physical injuries (caused by flying balls or my own questionable decisions).
• Emotional distress (caused by losing in tug-of-war).
• Property damage (e.g., my phone falling out of my pocket mid-relay race).
• Death (by embarrasment or otherwise).

I get it—fun comes with risks, and I won’t hold anyone accountable if I end up sore tomorrow.

By participating in this Adult Recess event, I understand that photos and videos may be taken to capture the fun, chaos, and occasional athletic glory. I grant Adult Recess the irrevocable, perpetual, and unrestricted right to use, edit, alter, and publish these images and videos in any form or format for any lawful purpose, including but not limited to:

• Social media posts (where my dodgeball skills—or lack thereof—might go viral).
• Marketing materials (because who doesn’t love a good action shot?).
• Promotional campaigns (to convince others to relive their childhood fun).

If I prefer not to be the star of the show, I can notify the organizer before the event. While efforts will be made to respect my request, I understand that accidental appearances in group photos or background shots are possible.

By signing below, I confirm that:

• I am 18 years old or older (and mentally prepared to play like a kid again).
• I have no medical conditions that would make this activity unsafe, like extreme dodgeball phobia.
• I understand this waiver, and I agree not to blame anyone but myself for my lack of athletic prowess.
