All families who are enrolled in Musikgarten and More Boca, LLC are agreeing to these terms and condotions.

By my signature below, I understand the instructor and location assume no responsibility for injuries or illnesses which I may sustain as a result of my physical condition or resulting from participation in a class. I hereby release and discharge the instructor and the facility from any and all claims for injury, illness, death, loss or damage, which may result from my participation in activities at this location.

I give my consent for images (photographs, video) of myself to be taken and used to document the activities of Musikgarten and More Boca, LLC. I grant Musikgarten and More Boca, LLC permission to use the images for educational and promotional purposes. By signing below, I agree that I have read and agree to the terms and conditions stated above.

**I understand that if I do not want images of myself or my child to be used, I will indicate this by checking the “I DECLINE” box below AND notifying Jessica King during the first class.

Coronavirus/Covid-19 Risks. The novel corona virus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited or discouraged the congregation of groups of people. Musikgarten and More Boca, LLC has put in preventative measures to reduce the spread of COVID-19; however, Modern Boca Mom cannot guarantee that you will not become infected with COVID-19. Further, attending activities at the facility could increase the risk of contracting COVID-19. By signing this Agreement,I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending classes and programs conducted by Musikgarten and More Boca, LLC, regardless of the location of such classes and programs, and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Musikgarten and More Boca, LLC facility may result from the actions. Omissions, or negligence of myself and others, including, but not limited to, the Released Parties (as defined below), and other activity participants.

 

• All fees are earned when session begins and are non-refundable

• Classes are non-transferable

• Session includes any/all classes you can attend. No makeups available. If class must be canceled due to heavy rain/thunder/lightning/tropical storm/hurricane/act of God, a custom virtual class link will be recorded and sent via email.

• 'Trials or Drop-ins' are only available for friends or family of a paid student who are visiting and want to attend class with their little ones: a la carte fee is $40.

• $17.50 sibling fee for any child attending with a paid student over the age of 6 months

• Prior to attending your first class the 'Liability & Covid Waiver' MUST be completed and submitted by a parent and/or a legal guardian to participate.
