Participation Acknowledgment
I understand that participation in certain activities offered at the Columbia County Health, Wellness & Fitness Expo — including but not limited to group fitness classes, health screenings, wellness demonstrations, and other physical activities — is voluntary and may involve physical movement or exertion.
I acknowledge that participation in these activities carries a risk of injury or health-related conditions, and I choose to participate at my own discretion.
Assumption of Risk
By attending and participating in any activities at this event, I voluntarily assume full responsibility for any risks, injuries, or health-related conditions that may arise as a result of my participation.
Release of Liability
I hereby release and hold harmless the Columbia County Health, Wellness & Fitness Expo, its organizers, sponsors, instructors, volunteers, partners, and the Columbia County Exhibition Center from any claims, liabilities, damages, or expenses that may result from my participation in event activities.
Medical Authorization
In the event of an emergency, I authorize event staff or volunteers to obtain necessary medical assistance on my behalf if needed. I understand that I am responsible for any medical costs incurred.
Media Release (Optional)
I understand that photographs and video recordings may be taken during the event for promotional and educational purposes.
