Ordering AND pickup will be open November 20th - November 27th, Monday - Friday from 7:30 am to 4:30 pm daily.

Each meal will serve 4-6 people. You can pickup your meals in our Teaching Kitchen within the Health and Wellness Center at Memorial Hospital.

Dear Participant,

Our team at Memorial Hospital's Health & Wellness Kitchen is dedicated to culinary education and is proud to have an outstanding facility that showcases great chefs/instructors and their cuisines. Our meal pickup service is dedicated to providing an outstanding food product for you to prepare and enjoy outside of our facility.

By signing this form, you acknowledge, agree, and understand that your participation is voluntary, and you assume the risks associated with meal pickup. You are solely responsible for, and assume all risks related to, the proper and safe preparation (including storage, washing, and cooking), use, and consumption of the contents of the meals. As such, all contents of the respective meals are provided “as-is.” Such risks may include, but are not limited to, physical injury, foodborne illness, allergic reaction (as the Health & Wellness Kitchen is not allergen-free), or other harm. You, and you alone, are solely responsible for taking proper care of meals to limit your risk of injury or harm.

Allergen Information. Please note that the nine major allergens, as determined by the US Food and Drug Administration, which are wheat, egg, soy, milk, tree nuts, peanuts, fish, shellfish, and sesame are stored, portioned, and packaged in our facility and our supplier’s facilities. You understand, acknowledge and agree that we store, portion and package these products, and while we take precautions to limit any cross-contamination, cross-contamination may occur between food products, and thus, the respective meals, may contain some or all of the allergens listed. You are also solely responsible for knowing about any food allergies you may have and verifying the products and their contents before handling, preparing, using or consuming such products.

Cancellations must be made no later than November 20, 2024 by 4pm in order to receive a full refund.
