When: Last 4 Wednesdays in July: July 8, 15, 22, and 29; 9:30 am - 2:30 pm

Cost: $150/Student

Limit 20 students (this will be the only round this year- please remember this camp always sells out so plan accordingly!)

Where: Shade Tree Nature School (Please map to Porter Community Center, Burnsfor travel time. We do not disclose address until forms are submitted for the safety of our students).

By putting your signature on these forms, they are binding and you are agreeing to all policies (including payment policies) set in our Parent Handbook. Please review thoroughly. The non-refundable tuition will be due on submission of forms with ABSOLUTELY no exceptions. Please venmo @ElliePropert or coordinate with Ellie (931-981-5811) on cash/check payment if you do not wish to pay through Venmo. Slots are not guaranteed until payment is received. If you submit forms without payment your slot(s) will go to the next person in line after 24 hours.

Shade Tree Nature School reserves the right to refuse enrollment for any reason and/or ask a family to leave the school should there be a breach in code of conduct and/or if our staff is not able to meet the needs of a student or family. 

This health history is correct and accurately reflects the health status of the student(s) to whom it pertains. The person described has permission to participate in all activities except as noted by me, the parent or legal guardian. I give permission to the school staff to call EMS if a situation requiring emergency services occurs involving my child.

If I cannot be reached in an emergency, I give permission to the emergency contact on my forms to make decisions regarding my child's well-being. If child's parent or emergency contact cannot be reached, I give permission to school staff to make decisions regarding my child's well being until I can be reached. I understand that school staff will discuss with me prior to enrollment my preferences surrounding my child's health.

I understand the information on this form will be shared on a "need to know" basis with Shade Tree staff. I give permission to photocopy this form. I have thoroughly read and understand the parent handbook and agree to abide by its terms. I acknowledge that he e-signature below is legally binding.

(Read Carefully Before Signing) This agreement by and between Shade Tree Nature School (hereinafter known as "Shade Tree") and the undersigned participant/trainer/instructor/employee/student/camper (hereinafter referred to as "participant") or the undersigned participants legal representative, parent, or guardian if such participant is a minor or does not have the legal capacity to enter into this contract is entered into for good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, and is subject to the following terms and conditions.

In consideration of being permitted to participate in any way in Shade Tree's tutorial, Camps, Programs, or Events and/or being permitted to enter for any purpose any restricted area (here in defined as any area where in admittance to the public is prohibited), the participant, named below agree: 

1. The parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating in Shade Tree, Tutorial, Camps, Programs, or Events, he or she should inspect the facilities and equipment to be used, and if he or she believes anything is unsafe, the participant should immediately advise the officials of such condition and refuse to participate. I understand and agreed that, if at any time, I feel anything to be UNSAFE, I will immediately take all precautions to avoid the unsafe area and REFUSE TO PARTICIPATE further. 

2.I/WE fully understand and acknowledge that: (a) There are risks and dangers associated with participation in Shade Tree, Tutorial, Camps, Programs, or Events which could result in bodily injury, partial and/or total disability, paralysis and death. (b)The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe. (c) These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named below. (d) There may be other risks not known to us or are not reasonably foreseeable at this time. 

5.I/WE HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Shade Tree, including its owners, managers, promoters, lessor of premesis, lessees of premises used to conduct Shade, Tutorial, Camps, Programs, or Events, and event inspectors, underwriters, consultants and others who give recommendations, directions, or instructions to engage in risk evaluation or loss control activities regarding the Shade Tree Tutorial, Camps, Programs, or Events held at such facility and each of them, their directors, officers, agents, employees, all for the purposes herein referred to as "Releasee" FROM ALL LIABILITY TO THE UNDERSIGNED, my/our personal representatives, assigns, executors, heirs and next to kin FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES OR DAMAGES AND ANY CLAIMS OR DEMANDS THEREFORE ON ACCOUNT OF ANY INJURY, INCLUDING BUT NOT LIMITED TO THE DEATH OF THE PARTICIPANT OR DAMAGE TO PROPERTY, ARISING OUT OF OR RELATING TO THE EVENT(S) CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEE OR OTHERWISE.

6.I/WE HEREBY acknowledge that THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage.

7.EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province or State in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.

8.On behalf of the participant and individually, the undersigned partner(s) and/or legal guardian(s) for the minor participant executes this Waiver and Release. If, despite this release, the participant makes a claim against any of the Releasees, the parent(s) and/or legal guardian(s) will reimburse the Releasee for any money which they have paid to the participant, or on his behalf, and hold them harmless.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. I acknowledge that the e-signature below is legally binding. 

This agreement by and between Shade Tree Nature School (hereinafter known as "Shade Tree")  and the party who signs, participating as a trainer, instructor, teacher, employee, volunteer, student or camper (hereinafter referred to as "participant") or participants legal representative, parent, or guardian if such participant is a minor or does not have the legal capacity to enter into this contract is entered into for good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, and is subject to the following terms and conditions.

I hereby grant to Shade Tree and their legal representatives and assigns, the right to photograph, videotape, and/or record my or my child/children's voice and sounds and to use any or all such photographs, recordings, and reproductions thereof in and/or as a part of any video production, broadcast, published products related to advertising, social media, trade show displays, and in any other purpose and in any manner and medium; and to edit the same without restriction.

I further grant the use of my name and/or my child/children's names in connection with my written or verbal comments and opinions. I hereby release Shade Tree and their legal representatives and assigns from all claims and liability relating to said photographs and video or tape recordings.

This release shall be binding upon the heirs and personal representative of the undersigned.

This e-signature is legal and binding.

Payment is due upon submission of forms. Please scan the provided QR code below before submitting forms. If you would prefer to pay by check/cash, please text Ellie at 931-981-5811 to coordinate drop-off of check. We do not disclose our address until enrollment forms have been completed for the safety of our students. Enrollment is not complete until payment has been received!

Address: Will be provided by email before school starts. This is a private residence and we value safety for both tenants and students, thus we do not share until we  have more information. 
What to bring- absolutely required without exception:
 1. A backpack with a change of clothes (including socks and underwear) and a towel 
2. Substantial snacks and lunch in a labeled cooler
3. Reusable water bottle (well-labeled) with only water. Extra beverages can be sent in lunchbox but please make sure they are dye/artificial sweetner free.
4. THIS IS REQUIRED FOR ALL STUDENTS- please remember that we are a nature school so the afternoon will be lunch/recess/free play after morning classes. We recommend you send your child in clothes that can dry quickly so they don't have to change before going home, and clothes that you are okay with them getting messy or dirty. Please wear comfortable shoes that you are okay with getting wet (rain boots recommended).

It will be hot, so please dress for the weather. Our parent handbook is sent out with our information packet and has tons of helpful information on dressing for the weather. We guarantee your child will have fun no matter the weather, and our campus is designed with weather changes in mind!

CODE OF CONDUCT

We value integrity, truth, and good character at Shade Tree. Our campus is large and has areas that require caution. It is of the utmost importance that your child obey rules completely for their safety and the safety of others. Here is what is not tolerated: 

Refusal to follow instructions
Cussing, foul language, inappropriate topics, intentionally excluding other participants
Bringing weapons to school
Leaving designated areas by oneself
Striking or threatening to strike any member of Shade Tree, student, volunteer, or staff
Abuse or threat to abuse school property
Hitting or striking animals
Disobeying school rules (reviewed thoroughly multiple times a day.)

Please understand that our mission is to protect all our students and we aim to make decisions that are fair and just. If your student breaks the code of conduct we will call you to come get them immediately. We will evaluate if they will be allowed to come back for the following weeks. Please view the policies on pages 18-20 of our handbook. You will not be refunded if your child is sent home.

If we are unable to proceed with class due to ice/snowy weather you will receive a full refund. This will be determined by the road conditions, wind, and temperatures below 10* F. Shade Tree Nature School will carefully consider all factors (If Dickson or Williamson county schools are closed we may still be open.)

Please coordinate with Ellie at 931-981-5811 if you need to pay by cash/check. By signing these forms you agree to pay the non-refundable tuition (due on submission of forms) even if you choose to withdraw from the program before your start date.
