✤ Please read the WHOLE form and take your time filling out all sections as clearly as possible. Upload relevant images as you go then give it a proof read before hitting ‘submit’. Forms with too many empty sections or vague information are hard to work with and unlikely to allow me to fully understand the type of tattoo you’re looking to have done!

✤ Some sections are required so that I can collect all of the necessary information for booking your appointment while others are optional.

✤ Please note: I will be prioritizing requests which appeal to the direction I wish to take my work.

✤ Classical fine art references from engravings, sculptures, book illustrations or iconic ceiling paintings.

✤ Mythical animals/creatures, alone or with ornamental surroundings.

✤ Religious iconography, sacred hearts, crucifixes, gothic and baroque architecture.

✤ Vases, empty or filled with foliage.

✤ Adaptations of work by any of the following or similar artists:

My rates are $180/hour plus NJ sales tax. By checking the box below you confirm that you are aware of my current rates.*

I’m requiring a $50 deposit to hold the appointment for you. This deposit is non refundable but transferable should we need to reschedule our session for any reason. By checking the box below you confirm that you are aware if I do not receive your deposit promptly after I send your scheduling text I will be forced to forfeit our agreed upon appointment date for another client.*

Please double check all sections and make sure you've provided as much information as possible.

I generally review my forms on Mondays {sometimes mid week if I have the chance}.

I will reach out via text message with further scheduling/deposit/waiver form information so please be sure to check any spam message folders your phone might have. If I do not receive a text response from you I will then contact you via your email. After these attempts I will assume you are no longer interested and therefore will not pursue you for further scheduling.

All bookings will be secured with a $50 deposit so please have this available to make securing your appointment as smooth as possible.

Deposits are non refundable but transferable should we need to reschedule for any reason and will go towards the final cost of your session.

I understand life happens and loads of things can come up unexpectedly. I'm willing to work with you but I can only accept one reschedule with the original deposit. If you should need to reschedule a second time I will require another $50 deposit.

I do my own scheduling without the help of an assistant. I'm usually really quick to respond, however, if you do not hear from me within one week of submitting your booking request please feel free to call the shop to ensure that I received it!

On the occasion that I am unable to fulfill your request I will be more than happy to advise you or recommend another artist within the shop who might be more suitable for your specific piece.
