According to the Center of Disease Control, (CDC) they do not recommend obtaining the vaccination more than one time in your lifetime. The vaccination is a series of three vaccinations within a six month time frame. After the third and last vaccine has been giving, there is no reason to obtain the vaccination again. I understand that due to my occupational exposure to blood or other potentially infectious materials, I may be at risk of acquiring hepatitis B virus (HBV) infection. I have been given the opportunity to be vaccinated with hepatitis B vaccine, at no charge to myself. If I choose to accept hepatitis B vaccination at this time, the EHS Director will issue an authorization form to obtain the vaccination at a facility in close proximity to the residents listed in the employee file. *The EHS Director will deliver the authorization form to your direct Supervisor for distribution. However, If I choose to decline the hepatitis B vaccination at this time I understand that I continue to be at risk of acquiring hepatitis B, a serious disease. If in the future I continue to have occupational exposure to blood, or other potentially infectious materials, and I want to be vaccinated with hepatitis B vaccine, I can receive the vaccination series at no charge to me.*
