Purpose: The purpose of this form is to provide chapter accountability and to comply with the Bylaws and IRS requirements.

Chapter Secretary should use chapter minutes to complete the report.

Data from this report will be shared with the S/P/N president and/or their S/P/N representative.

1. Chapter Meeting Information

Chapter Meetings. The purpose of the chapter meeting shall be to encourage fellowship among Alpha Delta Kappas; to share in programs related to the ideals and purposes of the organization; to conduct chapter business; to elect and install officers.

Chapters shall hold at least 9 meetings during the calendar year. However, members are not required to attend all 9 meetings.

2. Chapter Executive Board Meetings.

The executive board of the chapter shall be comprised of the elected chapter officers and immediate past chapter president.

The Executive board shall have a minimum of three (3) board meetings a year.
