3 Month Contract Agreement

AGREEMENT:  3-Month Membership Agreement for Boxing Gym & Fitness

 

This Membership Agreement (“Agreement”) is entered into between Put Em On Boxing & Fitness (“Facility”) and  (“Member”). The Facility agrees to provide access to its boxing gym and fitness areas to the Member under the terms outlined in this Agreement. By signing, the Member agrees to be bound by the terms and conditions stated herein.

 

Membership Term and Payment

 

The Member agrees to a three-month membership at Put Em On Boxing & Fitness, beginning on the date sign below and ending date 3-Months after. The monthly membership fee for this period is $35, due each month. The Member authorizes Put Em On Boxing & Fitness to charge their provided credit card $35 on the same date each month for three consecutive months. By signing below, the Member confirms that they understand this charge is non-refundable within the term of the Agreement.

 

Membership Benefits

 

During the three-month term of this Agreement, the Member is entitled to full access to the boxing gym and fitness areas at the Facility during normal operating hours. This membership grants access exclusively to the Member and may not be transferred or shared with any other individual.

 

Cancellation and Refund Policy

 

This Agreement is binding for the three-month term specified and is non-cancellable and non-refundable except in the case of Facility-approved circumstances, as described in the “Early Termination” section of this Agreement. The Member agrees that failure to utilize the membership benefits does not relieve them of the obligation to pay the monthly membership fee.

 

Renewal and Expiration

 

This membership will automatically expire at the end of the three-month term unless the Member opts to renew. In the case that the Member wishes to renew, a new agreement must be completed prior to the end date of this Agreement.

 

Early Termination

 

Early termination of this Agreement may be considered only upon written request submitted by the Member to the Facility’s management. Early termination is subject to Facility review and approval, which may include applicable fees for the remaining term balance. In the case of approved termination, the Member will be informed of the applicable cancellation terms.

 

Member Responsibility

 

The Member agrees to abide by all rules and regulations of Put Em On Boxing & Fitness. The Facility reserves the right to revoke membership, without refund, should the Member fail to comply with Facility rules or conduct themselves in a manner deemed harmful to the Facility, staff, or other members.

 

Liability Waiver

 

The Member acknowledges that participation in fitness and boxing activities involves certain risks. By signing this Agreement, the Member agrees to waive any and all claims against Put Em On Boxing & Fitness, including its owners, employees, and affiliates, for any injuries, damages, or losses that may result from using the Facility, its equipment, or services.

 

Acknowledgment and Authorization

 

By signing below, the Member acknowledges that they have read and understand the terms and conditions of this Agreement, and they authorize Put Em On Boxing & Fitness to charge the monthly membership fee of $35 to the provided credit card each month. The Member understands that these charges will recur monthly for three months and that this authorization remains in effect for the term of the Agreement.
