Risk Acknowledgement & Waiver Form

This is an important document and you must read it before signing.

To be completed by all:

1. Participation: I acknowledge and accept that the activities require a moderate level of fitness and can be physically challenging and I / he / she / they should not undertake the activities unless I / he/ she/ they am (is/are) physically able to. I acknowledge and accept that the activities are dangerous and there is risk of personal injury when undertaking such activities.

2. Personal Responsibility: In the unlikely event of an accident, loss or damage to any personal effects. I acknowledge Jungle Play will not be liable for any direct or indirect loss, damage or injury arising from or in connection with the activities (expect for death or personal injury caused by the company's negligence) and I waiver all and any claims against the company in this respect.

3. CCTV Monitoring: I acknowledge and accept that CCTV images of all entry and exits (including fire exits), all areas within the play centre and communal area within the premises will be monitored and recorded throughout the premises for the purpose of crime prevention and public safety.

4. Consumption of alcohol: I understand that consuming alcohol at the venue is optional and a personal decision. I agree to drink responsibility and I acknowledge that I am soley responsible for all my actions, including damage or injuries resulting from cosuming alcohol and Jungle Play or Jungle Play staff members are not responsible in any way.

5. Photo/Video release: I grant Jungle Play the right to take photographs or videos when the social media team wish to for promotional purposes without compensation. If I do not wish myself or my child to be involved I will kindly them or move away from the filming area.

Waiver Information

1. I am the parent/guardian of the child named above who is under 18 years of age. I wish for the child to participate in the play activities contained within the Jungle Play Limited located at 9 Green Lane, Formby, L37 7DJ, within 12 months of the date of completion of this form. I declare that if i am not the parent or guardian of the child I have authority from the child's parent or guardian to sign this risk acknowledgement & waiver form and disclose the child personal information.

2. I agree that I am responsible for the child names above who is in my care and undertake to ensure that he/she undertake the activities in accordance with the safety rules and advice that I and he/she recieve along with any oral instructions or advice given to me and them prior to the session. in addition to I agree to ensure the child wear socks whilst participating in the activities. 

3. I acknowledge that I am responsible for the safety and supervision of the child named above and the safety of your possessions. I will ensure that I will supervise them at all times.  

4. I certify that to the best of my knowledge the child do not have any medical conditions which might have an effect on them whilst conducting the activities according to the rules and regulations stated above.
