I understand that Storytelling Project is  a part of Wheaton College and seeks to serve  Christ and advance His Kingdom. I understand signing this authorization is voluntary, and that refusing to sign this form does not stop disclosure of information that is otherwise permitted by Federal law (FERPA) with or without my specific authorization, consent, or permission.

This authorization will remain active for 12 months from the date of signing. I understand that I may revoke this authorization at any time by contacting student.engagement@wheaton.edu.
