Please complete this questionnaire regarding your dog's behavior to the best of your ability. Let me know if you have any questions or anything to expand that I should know about as well so we can best identify your training goals in sessions!
About You
Most of this form will (no surprise!) be about your dog, but first give me a little information about you - not just your contact information, but your expectations and relationship surrounding your dog, your preferred coaching style, and a few other important details.

What training experiences have you had with pets in the past? What worked well? What didn't work well for you?
How did your current dog come into your life? What have you looked forward to doing with them? Do you feel like you get to experience life the way you'd hoped to with your dog?
*
About how much time do you realistically have available to spend during the week dedicated primarily to your dog?
*
What is your approximate "training budget"?
*
Training is an ongoing process, and goals are usually ever-evolving. Especially for behavioral challenges, progress is rarely linear and requires maintenance to be sustainable. Your commitment to continued engagement with your dog will be necessary for success.
*
I understand
How do you and your dog share joy together?
*
What are your current training goals for your dog?
*
As mentioned above, training goals are always changing and evolving. Goals may be revised and deemed unsustainable, or goals may be reached and exceeded and more ambitious goals can be set. Like Magic Dog Training believes completely in meeting the dog's and human's needs in any training scenario - which means that guarantees cannot be made because some methods of compliance do not bear in mind reasonable expectations for both human and dog.
Do your training goals most fit into working with:
*
Life Skills
Behavior Challenges
Puppy Raising
Adolescent Manners
Kids and Dogs
Are you most interested in:
*
Character Creation: Private training packages with you and your dog building skills together and working on behaviors between sessions
MultiClass: Weekday board and train program with weekly handoff sessions to keep you involved in the learning process but taking some of the practice off your shoulders
Campaigns: Board and train packages intended to provide behaviors solutions that are transitioned to you in coaching sessions once the behavioral foundations have already been built

Contact and Care
This section is specific to dogs who may be staying overnight with Like Magic. Please complete if you may be interested in boarding or board and train programs and packages!
Are there specific dates you are hoping to schedule a board and train stay?
My dog has experience and is comfortable around:
People (Adults)
People (Kids)
Other Dogs
Cats
If your dog actively *dislikes* (rather than just doesn't have experience) interaction with the above, please tell me about that here!
My household is made up of my wife, two kids, three dogs - as well as other boarders - and two cats (and two snakes, but they don't interact with dogs ever!). Interaction with any of these beings is entirely voluntary based on your dog's comfort and preference. Provided they can comfortably crate in the home with them, direct interaction with any of my household members is not a requirement of their stay.

Is your dog currently on any behavioral medication? If so, what medication are you using and what impact has it had on the problem behaviors thus far?
Behavior History
There is so much that goes into your dog's personality, temperament, and behavior that happened long before they came into your life, even if you brought them home as a tiny 8 week old puppy. While rescues can have a more unclear history, please share any details that you are aware of about your dog's background.
Does your dog share the house with other pets? Please include names and ages of non-human family members and how they get along with your dog
*
Would you say your dog is an introvert or an extrovert? Why?
*
What past experience does your dog have with training? What behaviors have worked well? What approaches have not worked?
Has your dog ever put teeth on a human? (Please check all that apply)
*
No, not to my knowledge
Yes, their teeth have touched skin without a puncture. They may have left a red mark or bruise/scratches
Yes, they have punctured the skin in 1-3 places with a single bite - no tearing or slashing.
Yes, they have caused 2-4 punctures from a single bite with considerable bruising and tearing/slashing.
Yes, they have caused 2-4 punctures from multiple bites with considerable bruising and tearing/slashing.
Please provide details of any incidents where teeth made contact with a human, including any preceding triggers, and if medical care was necessary for the incident.
Behaviors Today
I'd like to understand as best as possible what situations you are seeing your dog's undesirable behaviors arise and how both dog and human react to those situations. Ideally, behaviors we will work on will prevent the undesirable behavior from happening at all, so knowing how to best get ahead of it is vital to our training plan!
How long have the behaviors you're looking to work on been going on? Has it gotten better or worse over time? What has changed?
*
Especially in terms of behavior concerns (as opposed to manners), knowing how long they've been going on provides information about what habits and reinforcement history we're looking to adjust, but also might point to other concerns. Behavior problems that show up suddenly out of seemingly nowhere - especially in an adult dog - may have medical or pain components. My recommendation in situations where problem behaviors suddenly pop up is always to have your vet on board (often as well as a trainer) to assess any potential health concerns.
What, if anything, precedes your dog's concerning or challenging behaviors?
*
This can be environmental triggers that you've noticed or body language from your dog. Can you predict when the problem behavior is about to occur?
How does your dog behave after the concerning/challenging behavior?
*
Is your dog able to recover - either through engagement with you, or settling down. Do you see the behavior persist as long as the "trigger" is in sight/in earshot/in your home. What does "recovery" look like for your dog?
What emotion(s) do you feel are driving this behavior?
*
Excitement
Frustration
Anger
Anxiety
Fear
Conflict
Possessiveness
Impulsiveness
Not Sure
Other
What makes you think your dog is expressing the emotion(s) you chose?
*
Does your dog struggle with excitement, fear, or reactivity surrounding:
*
People in the home
People outside the home
Dogs in the home, off leash
Dogs outside the home, off leash
Dogs on leash
Dogs behind fences or barriers
Cars and Trucks
New Places or Experiences
If they struggle with any of the above, what does that behavior look like? How close can the "trigger" be before it results in a reaction?
