- We appreciate your interest in performing at 561 Music Festival 4.
- This form is for the April 5th, 2025 performance opportunity only.
- Completion of this form does not guarantee a performance slot for the festival.
- Application Deadline: December 20, 2024.
- All Performance Acts will be reviewed, and you will be notified as to your status for the event no later than January 13, 2025.
- Bands MUST have original music, no cover bands please.
- Only one act per application, please.
- Please only submit once per band.
