Use this form to submit your ORIGINAL band for consideration to perform on one of two stages at 561 MUSIC FESTIVAL 5, Saturday May 2nd 2026, Meyer Amphitheater, on the West Palm Beach Waterfront.
- Open to any ORIGINAL bands based in Florida
- This form is for the May 2nd, 2026 performance opportunity only.
- Completion of this form does not guarantee a performance slot for the festival.
- Application Deadline: December 15, 2025.
- All Performance Acts will be reviewed, and you will be notified as to your status for the event no later than January 15, 2026.
- Bands MUST have original music, no cover bands or tribute bands please.
- Only one act per application, please.
- Please only submit once per band.
