As a parent I understand and acknowledge that participation in any sport event involves fundamental risks, including but not limited to bodily injury, property damage, and other dangers associated with it. As a parent, I freely and voluntarily consent for my minor child or children to participate in sports physical activities with Elite Youth Ballers Foundation. I have had the opportunity to read this waiver carefully and opted for consultation by an attorney as required by law.
As a parent, heirs, executors, and assigns of my child or children, I agree with the risks my child or children are undertaking by participating in Elite Youth Ballers Foundation activities and voluntarily wave all my rights and or claims against Elite Youth Ballers Foundation, its affiliates, staff, representatives and third-party organizations. I understand this waiver includes any risks associated with travel to and from the facility or participations, including but not limited to physical or psychological injury, illness, disability, economic loss, or death.
As a parent, I authorize Elite Youth Ballers Foundation, including its officers, directors, employees, volunteers, agents, third-party partners, and representatives, to contact 911 for necessary treatment as required by law and to notify me as soon as possible in the event of an emergency involving my child or children. Also, I understand my child or children are allowed to participate in the Elite Youth Ballers Foundation activities as long as my child or children do not engage in verbal or physical abuse with any personnel player or coach in its organization or affiliates third-party. I further acknowledge that my child or children have health insurance and or both liability insurance policies.
As a parent, I hereby grant Elite Youth Ballers Foundation a limited license to use my child’s or children’s name, image, voice, likeness, attributes, and biography for the purposes of advertising, publicizing, and promoting Elite Youth Ballers Foundation and its related activities. This includes, but is not limited to, the creation, distribution, and monetization of television programs or other video content featuring footage of my child or children, whether broadcast on television, online, over the internet, or through any other media platforms.
Under this agreement as a parent, I hereby release, waive, discharge, and covenant not to sue against Elite Youth Ballers Foundation, its officers, directors, employees, volunteers, agents, third party organizations and representatives from any or all claims, liabilities, damages, losses, or expenses, including but not limited to medical expenses, arising out of or in any way related to my child's or children’s participation in the sports activities or events. I further understand that as a parent or legal guardian I can opt out of this agreement at any time by writing to Elite Youth Ballers Foundation in email or text message.
By signing below, my child or children and I understood the risk and responsibilities we must adhere to in this term’s agreement. Henceforth, as a parent or legal guardian, I agree in full to this liability release and willingly waiving any right I have to pursue any legal action against any personnel within Elite Youth Ballers Foundation or third-party organizations for any injury or property damage involving me or my child/children.
