CB Uniform Order Form

Please fill in all required fields and tick the boxes for each item you wish to order. Only items with checked boxes will be included. For full uniform details, please refer to the Uniform Requirements Booklet to ensure you select all required items for your classes. Please note that 2026 marks the first official year of our new uniform and updated logo, and therefore we do not yet have photos of the finished garments. The Wear Moi Iliade and Penelope leotards, along with all modern uniform items, will come screen-printed with the CB logo, while the CB Puffer Vest will be embroidered.
BALLET UNIFORM (Wear Moi Items)

Please select the leotard colour and size required for your level, and refer to the Uniform Requirements Booklet for acceptable warm-up items. Photos of each leotard colour can be viewed by clicking on the image and scrolling through.
MODERN UNIFORM & CB BRANDED ITEMS

For all Modern classes, students are required to have a minimum of one top option (e.g., crop top) and one bottoms option (e.g., bounce bike shorts), as well as any relevant warm-up items (e.g., hoodie/track pants). Only Canterbury Ballet uniform warm-ups may be worn—non-uniform items are not permitted in class.
