1. General Consent

I, the undersigned, consent to receive the services provided by Beauty+ with Christina, including but not limited to lash services, waxing, airbrush tanning, and other beauty treatments (collectively referred to as “services”). I understand that these services are entirely voluntary.

2. Acknowledgment of Risks

I acknowledge and understand the following:
• Lash Services: There may be risks of irritation, allergic reactions, or damage to natural lashes due to adhesives, materials, or techniques used.
• Waxing Services: Waxing may result in temporary redness, irritation, or, in rare cases, burns, bruising, or ingrown hairs.
• Airbrush Tanning: Tanning products may cause skin sensitivity or staining. I understand I must follow pre- and post-care instructions for optimal results.
• Other Services: Additional risks will be disclosed before receiving specific services.

I understand that Beauty+ with Christina is not liable for any adverse reactions or outcomes due to pre-existing conditions, allergies, failure to follow instructions, or circumstances beyond their control.

3. Health Conditions

I confirm that:
• I have disclosed any allergies, sensitivities, or medical conditions that may affect my ability to safely receive services.
• I have not recently undergone chemical peels, laser treatments, or other procedures that might affect the treated areas.
• I am not currently experiencing any contagious skin conditions, open wounds, or illnesses that may pose a risk to others or interfere with services.

4. Release of Liability

I release Beauty+ with Christina, its owners, employees, and contractors from any liability, claims, or damages arising from the services provided. I assume all risks associated with the treatments.

5. Photography and Marketing Release (Optional)

I consent to Beauty+ with Christina taking photographs of my results for promotional purposes, such as social media or marketing materials.

6. Cancellation and Refund Policy

I understand that cancellations or rescheduling must be made 12hrs in advance. Missed appointments may result in a cancellation fee or loss of deposit. Refunds are not provided for completed services.
