I am the natural parent, guardian, or managing conservator of the above-mentioned child, a minor. I hereby give permission for my child to participate in the following activities of First Baptist Church Graham, Texas (FBCG) - First Kids Ministry, August 1, 2024 - July 31, 2025.

In consideration for my child being permitted to utilize the facilities, services, and programs of FBCG in the above-listed activities, I agree to RELEASE, WAIVE, INDEMNIFY, HOLD HARMLESS, AND DISCHARGE FBCG, its officers, employees, agents, representatives, successors, assigns, and volunteers, and all persons, natural or corporate, in privity with them or any of them, from any and all claims or causes of action of any kind whatsoever (including causes of action for negligence or gross negligence), at common law, statutory or otherwise, that the child, the child's parents, the child's legal representatives, heirs and/or assigns, have or might hereafter have, known or unknown, now existing or that might arise hereafter, directly or indirectly attributable to the child's participation in First Kids Ministry, August 1, 2024 - July 31, 2025.

Further, should my child need medical treatment while attending the activities of FBCG, I grant the authority of FBCG, its officers, employees, agents, representatives, and volunteers to consent to medical treatment in the event I cannot be contacted. This authorization expressly includes the authority to sign releases on my behalf for medical services and facilities. I promise to assume liability for payment of all such medical services and facility fees and to reimburse FBCG for any medical expenses that may be incurred on behalf of my child.

Visual Production Constent & Release
First Baptist Church Graham, Texas, and its First Kids Ministry (FBCG) occasionally create ministry videos, publicity, website, and/or public relations advertisements (“Visual Productions”) as a part of the work of FBCG. While FBCG is under no legal obligation to obtain a release for your child to be a part of these Visual Productions (nor is FBCG assuming any legal obligation arising out of this Consent and Release), FBCG is attempting to affirmatively record your approval for your child’s participation in these Visual Productions. We request that you sign this form to FBCG with respect to your child.

The agreement below allows you to express your approval for your child’s name, picture, art, written work, voice, verbal statements and/or portrait (video or still) (collectively, “Image/Statement”) to appear in FBCG Visual Productions. These Visual Productions may or may not personally identify your child. The Image/Statement may be used by FBCG in Visual Productions in subsequent years.
CONSENT AND RELEASE
The undersigned Parent/Guardian releases to First Baptist Church Graham, Texas the Image/Statement of the child listed below, and the undersigned consents to FBCG’s use of the Image/Statement as a part of FBCG Visual Productions.
