        Camp groups are divided into Ages: 4 - 6, 6 - 8, 8 - 11. WE LIKELY CANNOT ACCOMMODATE WIDE AGE/DEVELOPMENTAL GAPS (e.g. a 5 year old with a 7 year old). Each age group comes with unique interests and abilities. We most often accommodate all friend/sibling requests that are *reasonably* possible. We cannot guarantee your child will be in the same group with all their friends/siblings if there are too many friends/siblings listed, WIDE AGE GAPS, past examples of challenging/unsafe behaviour when they are together, or anything else. Please make a note if you want to prioritize siblings being together over any friend requests. Please do not email any friend/sibling requests if you have already listed them here. We will see them!!

      • Permission Forms and Waivers

        The following permission forms and waiver apply to my child(ren), whose names are listed on this registration form as Campers 1, 2, 3, 4, to attend Tamarack West March Break Camp.
      • Permission to Seek Medical Assistance

        In case of a medical emergency and I, or my provided Emergency Contacts, cannot be reached first, I hereby give permission to a staff member of Tamarack West March Break Camp to seek medical assistance and treatment as may be deemed necessary.
      • Refund Policy

        !! NEW SINCE 2024: The amount of cancellations in the prior year created a financial burden to the camp and also resulted in reduced work hours for our staff. We hate to do this, but we need to update our refund policy to protect the camp and our commitment to our staff. 50% refunds are accepted up to 28 days prior to the first day of camp. In other words, there will be a 50% penalty fee for cancellations made from the day you register up to 28 days before the first day of the camp session. Refunds will not be issued should a cancellation be made: (1) to a reservation within 4 weeks (28 calendar days) of the start of your registered session, and (2) to a reservation once your child(ren)'s registered session has already begun.
      • Tamarack Behaviour Policy

        The safety and well being of all campers and staff is our utmost priority. We ask our campers and staff to make sure in all their actions that they are caring for themselves, taking care of others, and taking care of their surroundings. This community well-being approach, is mentored through encouraging empathy and consideration. If and when conflict arises, we take the time as a group, to address the issues and look for resolutions.
      • Camper Dismissal Policy

        Our goal as a camp is to help all campers realize their full potential and enjoy a great March Break camp experience. Part of this is making sure everyone feels safe and supported. Depending on the degree of concern, a camper who threatens their own safety or the safety of others could be required to make a formal agreement with the group in general, which if broken could lead to limited participation or complete dismissal from the camp. In the case of a more serious concern, the camper may be asked to leave immediately.

        I hereby give permission for my child(ren) to take part in field trips with Tamarack West March Break Camp involving walking or public transit in the Toronto area during March Break Camps.

      • Acknowledgement of Policies

      • Tamarack West March Break Camp Waiver

        AGREEMENT: Including Informed Consent and Assumption of Risks, Waiver of Claims, Release of Liability, and Authorization to Obtain Medical/Emergency Services (hereinafter collectively called the “Release Agreement”), for participation in activities associated with TAMARACK WEST OUTDOOR SCHOOL INC. (the “Company”).

        Please Note: By signing this Release Agreement, you will WAIVE CERTAIN LEGAL RIGHTS, including the RIGHT TO SUE OR CLAIM COMPENSATION FOLLOWING AN ACCIDENT, and you will ACCEPT LEGAL OBLIGATIONS. Please Read Entire Agreement Carefully!

        ACTIVITY: In this Release Agreement, “Activity” shall include all activities in any way related to, but are not limited to: tree climbing, hiking, nature studies, wilderness exploration, wildlife viewing, animal identification, proximity to and/or contact with live or deceased animals, animal tracking, wading, carving, sawing wood, woodworking, craft making, use of camp stoves and campfires, fire building, cooking over an open fire, instructional courses, use of tools, food and beverage, water supply, stewardship activities, plant identification, contact with plants, consumption of plants, hiking, shelter building (from any material including, without limitation, snow, sticks, mud, and other debris), ice skating, sledding and/or tobogganing, parkour; transportation or travel to and from the school , field trips, or any other destinations, travel using public transportation, library visits, interacting with community members, and all other activities offered (hereinafter referred to as the “Activity”)

        I (the “Undersigned”), hereby sign this Release Agreement on behalf of myself, my personal representatives, heirs and assigns.

        The person participating in the Activity with the Company, either the Undersigned or the Minor (defined below), is hereinafter referred to as the “Participant”.

        If the Participant is a minor, the Undersigned is the Parent/Guardian of the participant (the “Minor”) and executes this waiver on behalf of the Minor, the Minor’s personal representatives, heirs, and assigns, in Undersigned’s capacity as Parent/Guardian and with the intent that this waiver be binding on the Undersigned and the Minor for all legal purposes.

        The Undersigned hereby agrees as follows:

        1. RELEASE. The Undersigned agrees to release, and hereby does release, the Company, their respective directors, principals, teachers, officers, agents, employees, volunteers, contractors, affiliates, and representatives (collectively, the “Releases”), from any and all liability or claims that they or any of them may have now or in the future, for any personal injury, death, property damage or other loss, sustained by the Participant in connection with, or as a result of, the Participant’s participation in the Activity, due to any cause whatsoever.

        2. ACKNOWLEDGEMENT OF RISKS. The Undersigned acknowledges that there are certain risks involved in participation in the Activity, in addition to the usual risks and dangers inherent in any physical activity, these risks include but are not limited to: exposure to inclement weather; slipping; falling from a height; steep slopes in their natural state that may contain many obstacles and hazards; rock slides; insect or animal bites; cuts; head injuries; encounters with domestic or wild animals; being struck by falling objects; hypothermia due to exposure to very cold weather and/or weather conditions; medical conditions related to extreme heat; uneven terrain; stream crossings; terrain that may not have been travelled on or climbed before or is not regularly patrolled or inspected; becoming lost or separated from the teacher or party; actions involving building and maintaining a fire; entrapment by trees, logs, rocks, tents, or equipment; travel near active roads; travel near pedestrian traffic; travel near wheel scooters and other mobility devices; encountering persons on the street; social or economic losses; loss or damage of personal property; injury; permanent disability; or fatality.
        
In addition to the hazards and risks described in the paragraph above, the hazards and risks of the Activity may also include, but are not limited to the following: latent or apparent defects, malfunctions, or problems in equipment provided by the Company or outside service providers; negligence of other participants; physical contact with another person; weather conditions (including unforeseen, inclement or intemperate weather); consumption of food and drink; fire; first aid; emergency treatment; or other services rendered. Further, the Company will not always have medical personnel (other than instructors that are certified in first aid) at the location of the Activity.

        3. ASSUMPTION OF RISKS. The Undersigned and on behalf of the Minor, freely accepts and fully assumes all such risks, dangers and hazards connected with, or related in any way to participation in the Activity, and the possibility of injury, illness, death, property damage or loss, resulting from any of the foregoing.

        4. AGREEMENT. We understand and agree that participation in activities associated with the Company requires the Participant to:
        • Share the responsibility for the safety of their self and others during all activities.
        • Follow all instructions and directions of the Releases. Failure to do so may result in removal from the program.
        • Assume the above risks and accept responsibility for all damages and loss resulting from their participation.
        • Arrive fully prepared (including, but not limited to, clothing and equipment) to participate in the Activity. The Undersigned accepts full responsibility for inadequate clothing or equipment and for clothing and equipment which they fail to provide.

        
5. REPRESENTATION OF FITNESS. The Undersigned acknowledges that the physical exertion required by the Activity and the forces exerted on the body can activate or aggravate pre-existing physical injuries, conditions, symptoms, or congenital defects.
        The Undersigned has been advised to seek medical advice if they know or suspect that the physical condition of the Participant may be incompatible with the Activity.
        The Undersigned certify that the student is fully capable of participating in the Activity.
        The Undersigned represent that the student is in good health and physically fit and has not been advised by a physician not to participate in arduous physical activities.
        The Undersigned knows of no reason, health-related or otherwise, why the student is not capable of participating in the Activity.

        6. NO REPRESENTATION. In entering into this Release Agreement, the Undersigned confirms that they are not relying on any representation or statement made by the Company or any of the Releases, whether oral or in writing, to induce participation in the Activity or minimize the risks or dangers connected with or related to their participation in the Activity.

        7. BINDING AGREEMENT. The Undersigned agrees that this Release Agreement constitutes a legally binding obligation enforceable in accordance with its terms, and that it shall continue to bind the Undersigned, their heirs, executors, legal representatives and personal assigns.

        8. FACSIMILE: The Undersigned agrees that this Agreement may be executed by facsimile or digital transmission and that facsimile and digital signatures will evidence the obligation of the parties to be bound by the terms of this Agreement.

        9. GOVERNING LAW. This Release Agreement will be governed and interpreted in accordance with the laws of the Province of Ontario and the law of Canada where applicable.

        I confirm that I am 18 years of age or older, that I have completely read this Release Agreement and that I fully understand the provisions of this Release Agreement. I understand that by signing this Release Agreement I am giving up my legal rights including the right to sue.

        IN WITNESS WHEREOF the Undersigned has duly signed this Release Agreement.

      • Parent/Guardian Health and Safety Agreement On Safe Practices to Prevent the Spread of Infectious Diseases (including COVID-19) at Tamarack West March Break Camp

        (as required by Toronto Public Health, Ontario Ministry of Health, and Ontario Ministry of Education)

        By signing this document electronically, I agree to perform the following in order to keep my child, all other camp participants, parents/guardians, and staff safe from infectious disease (such as COVID-19) while attending Tamarack West March Break Camps:

        I agree to have my child stay home and not attend camp if my child is sick and/or is exhibiting signs and symptoms of illness.

        I agree to screen my child every morning for signs and symptoms of COVID-19 before arriving at camp. This includes daily temperature checks for signs of fever (37.80C or higher), and performing this online self-assessment on their behalf: https://covid-19.ontario.ca/self-assessment/ or answer the questions on this screening form daily: https://www.toronto.ca/wp-content/uploads/2020/05/95f0-Survey-Screening-poster-TPH.pdf 

        By dropping my child off at camp, I confirm that I have completed the required daily screening whether electronically or on paper, in addition to a daily temperature check, and that my child does not show signs or symptoms of illness.

        I am aware that I will be asked immediately to bring my child home if they are ill upon arrival to camp.

        I am aware that I may be contacted during the day to bring my child home if my child is showing any symptoms for COVID-19.

        If I suspect that my child could have COVID-19, I agree to keep my child at home and not attend camps until completing any isolation guidelines advised by public health authorities. (I can refer to Toronto Public Health for isolation guidelines: https://www.toronto.ca/home/covid-19/covid-19-what-you-should-do/covid-19-what-to-do-if-you-have-covid-19/#:~:text=Stay%20at%20home%20until%20you,COVID%2D19%20testing%20and%20treatment.). 

         

        By my electronic signature below, I acknowledge that I have received, read to completion, understand, and agree to the Parent/Guardian Health and Safety Agreement. 

      • Camper Media Release Consent Form

        I, the parent/guardian, hereby agree and give my permission for Tamarack West March Break Camp ("Tamarack") to record, film, photograph, audiotape or videotape my child(ren)'s name, image, student work, and performance (hereinafter collectively referred to as “Works”) and to display, publish or distribute these Works for the purpose of publishing, posting on the Tamarack website, posting in the school, posting on Tamarack social media sites and/or for broadcasting on television or radio as determined by Tamarack. I hereby waive any right to approve the use of these Works now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Works. I understand that the Works may appear in electronic form on the internet or in other publications outside of Tamarack's control. I will not hold Tamarack responsible for any harm that may arise from such unauthorized reproduction.
