Disclaimer
The information on this form is true and correct. I agree to pay a one-time consultation fee. I understand that:

1. Payment of fee does not constitute the hiring of the lawyer in this matter;
2. The entire scope of the lawyer’s services is limited to the advice given during the initial consultation;
3. The lawyer does not represent me unless I sign a written retainer agreement and pay the initial retainer fee;
4. My matter may be subject to time limitations, that I may be barred from taking any action if I do not act in a timely manner, and that I am not relying on the lawyer to take any action.
5. If the initial consultation includes a review of documents, then I understand that the lawyer is performing a cursory review which does not give him/her enough information to advise me of all problems or potential liabilities that the document may present.  They will only provide me with a full analysis of the document after we have reached agreement concerning the scope of analysis and I have formally engaged them to do that work.
6. A hold amount equivalent to one hour of work (which includes review of documents and the consult time), plus applicable taxes, will be placed on your credit card. Final time and invoice will be provided upon completion of consult and although rare, any time spent over one hour, will be billed at the lawyer's regular hourly rate. Except where valid membership with partner associations is provided.
7. In accordance with Canadian privacy laws, including the Personal Information Protection and Electronic Documents Act (PIPEDA), we are committed to safeguarding your personal and financial information. Your credit card information will only be used for the purposes outlined in this authorization, and will not be shared with any third parties without your explicit consent, except where required by law.
