Parent/Guardian Responsibilities

I/We will inform Southern California Tennis Academy (SCTA) about any serious medical condition or disability of the learner, either existing or arising.

I/We will inform SCTA immediately of changes of address or telephone number of parents/guardians.

I/We agree that we and our child/children will follow all SCTA policies and regulations.

I/We agree to settle all financial obligations as they come due. All programs are prepaid at the beginning of each enrolled session

RELEASE AND WAIVER OF LIABILITY

In consideration of the use of facilities, transportation program, and participation in programs and events provided by Southern California Tennis Academy (SCTA) El Dorado Park Tennis Center (ELDO), and BTG Management (BTG), I/we agree to release, indemnify, defend and hold harmless SCTA, ELDO, BTG and their directors, officers, employees and agents (hereinafter ”Indemnified Parties”) from any loss, liability, claim action, or suit (hereinafter “CLAIM”) arising or alleged to arise from bodily injury or personal injury suffered by any party or Indemnified Parties’ facilities; any bodily injury or personal injury caused by a participant in Indemdified Parties’ programs and events (including off-site trips) and/or in transportation provided by Indemnified Parties'; Property damage or loss suffered in connection with, or as a result of, my use and the Player’s use of the facilities or participation in Indemnified Parties programs and events (including off-site events). I/we agree that I/we knowingly and voluntarily release and indemnify the Indemnified Parties from any such claim.
