3rd Grade Permission Slip: Georgia Aquarium

Please fill this form out before your scholar attends the trip.
A field trip is scheduled for our 3rd-graders to vist the Georgia Aquarium located at 225 Baker St NW, Atlanta, GA 30313, USA. Please keep the follwoing in mind:
- Trip Date: Wednesday, April 15th
- Trip Cost: $25.00 (per scholar)
- The group will depart at 9:00 AM and is estimated to return at 1:00 PM.
*DIA will not accept verbal approval*

**This form, and trip payment, must be completed by the parent/guardian. Payment can be made in Infinite Campus**
As the parent or guardian, I confirm that I have reviewed the details provided above. I agree to make sure the associated cost is paid in full. I grant permission for my child to participate in the enrichment field trip mentioned and acknowledge that my child will travel to and from the specified location by bus. Additionally, I authorize any necessary medical treatment in the event of an emergency.
