2025 Orange Field of Valor Flag Tribute Order Form
THREE-YEAR FLAG TRIBUTES AVAILABLE FOR 2025, 2026 AND 2027! When you get to the payment submission form, you will have the option to purchase a 3 year renewal of your Flag Tribute for a discounted rate of $30/flag per year ($90 for a three-year renewal). Your Flag Tribute will be automatically renewed in 2026 and 2027--you will not need to resubmit a form unless information has changed.
Please note, if you do not have all the information we are asking for, it is okay! You can still submit a form, providing as much information as you can. Please note that once you begin filling out this form online you cannot save it for later, so please gather your information before you start. If you need to make corrections after you submit, please email susie@communityfoundationoforange.org.
