Under application to adopt, you agree that members of the rescue may speak to your references and your veterinarian. You agree to allow the rescue to receive medical records and history of any animals under your care, as well as speak with the veterinarian(s) relative to the records.

Sterilization: If not already altered, I agree to schedule surgery to spay or neuter as soon as a licensed veterinarian feels this dog is of age and fit for said procedure. While waiting for said surgery, I agree that no breeding shall occur and agree to take steps to prevent breeding from occurring. Upon completion of altering surgery I agree to provide documentation to the rescue. I understand that breeding this rescue dog is strictly forbidden. I understand violation shall mean the immediate revocation of this agreement and return of this dog to the rescue. I understand that if permanent surgery to prevent reproduction is not performed, the rescue reserves the right to void the adoption and reclaim possession of the dog immediately, without any return of adoption fees or donations. New recommendation for reduction of blood/bone cancel required for later altering age. We recognize recommendation to prevent illness, however no breeding is allowed

This rescue understands the emotional needs of a rescue dog. The dog will require time to adapt and adjust. This is usually a minimum of four weeks, sometimes longer. During this time, be especially aware of the rescue dog’s potential to take “flight”. Dogs should be supervised, even in fenced yards, on outdoor breaks. Dogs should never be left outside without supervision in large part due to increase in dog theft.

When transporting newly adopted dogs and throughout their transition period, it is recommended that leads with handles be looped to your wrist. Double leads are recommended. It has been found that “martingale” collars do not slip off necks as easily as others. A halter harness is another safety tool to prevent flight.

I understand it is required that a rescue dog needs to be secured in a crate, or by seat belt anchor, while transporting. Concern is security for dog’s safety. When crates are in use while unsupervised, collars should be removed to prevent accidental hanging.

This rescue requires our dogs to be treated and live as family members. We do not allow dogs to be isolated. We do not allow chaining as a means of security and housing. Dogs are to be protected from the elements. The dog’s health and safety is to be considered paramount at all times. Adopter shall provide wholesome food, fresh water, and proper shelter in the family unit as a full member of said adopter family as referenced herein

This dog is to be placed on all recognized preventative health care including updates on vaccines, titers (if desired or needed), fleas, ticks and heartworm medication as prescribed by a local licensed veterinarian. In order to maintain optimum health for the life of the dog, regular checkups by a licensed veterinarian are required. By signing this agreement, I agree to these provisions and shall comply with same.

At no time is this rescue dog to be turned in, given, or sold to another rescue, shelter or other organization or person. This dog shall not be transferred to any other person, business or organization for any reason whatsoever.

If for any reason you are unable to keep this dog, your rescue representative should be contacted immediately. Arrangements will be made for the dog. There will be no compensation of any kind to the adopter or adopter’s representative. In the event this dog needs rehoming, you will be required to remain an “in home” foster for the dog while a new home is found. Cost of transport shall be by original adopter to the new home once approved by this rescue.

All rescue dogs are to be current with your county and state laws. Safe collars with tags are to be utilized. Please be aware collars should not be used while dog is unsupervised in a crate, hanging is a concern

Introduction to other dogs needs to be done slowly and in a protective manner. Do not leave dogs newly introduced alone together, until such time as they are fully integrated into the family

This rescue dog will not be trained in any aggressive manner. This rescue dog will not be trained for protection or as an attack dog, or otherwise trained to be aggressive for any reason. Training programs are to be submitted and approved for acceptance by your rescue representative

It is recommended that all dogs be given basic obedience classes. Prior approval of a training course is required. Purpose: many programs use non-recognized training methods that can instill anxiety and insecurities. Our goal is to assist in courses that are recognized to provide good mental stability and positive outcomes.

This rescue reserves the right to follow up on this dog to check on its welfare and safety, including making home visits by appointment . If for any reason we feel the dog’s health, physical, mental, or emotional safety is deemed to be in question, we reserve the right to revoke the adoption agreement and take custody of this dog. As the adopter, you agree to notify your rescue representative if you move and to provide a current address within two weeks of relocating. As the adopter, you agree to provide positive training methods

Upon adoption of this dog, I understand I am expected and required to provide all ongoing needs. This includes monthly preventions and routine exams as well as sick or injured veterinarian needs. Proper food and nutritional needs must be provided.

Should it be necessary for this rescue to take legal action to recover this dog or otherwise enforce provisions in this agreement, the undersigned adopter agrees to pay all court costs and legal fees.

Should the adopter choose to bring a suit against this rescue or rescue representative for any reason whatsoever, adopter hereby agrees to waive the right to bring suit in court and agrees to be bound by the arbitration process as defined by the American Arbitration Association (AAA). Any such arbitration shall be governed by the rules of AAA and the State of Wisconsin.
