PHOTO/VIDEO CONSENT: I hereby grant Tiger Cho's World Class Taekwondo permission to use photos or videos that may include my child's image, likeness, or voice, captured during the Hickory Creek Elementary Enrichment Program for promotional and marketing purposes.*

LIABILITY WAIVER: I hereby acknowledge that my child is voluntarily participating in the Tiger Cho's Taekwondo Afterschool Enrichment Program at Hickory Creek Elementary. I understand that the nature of the sport/activity involves inherent risks, including but not limited to physical injury, illness, and property damage, and I willingly assume all such risks. In consideration of being allowed to participate in the Tiger Cho's Taekwondo Afterschool Enrichment Program, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Tiger Cho's Taekwondo, its officers, directors, employees, volunteers, agents, and representatives from any and all claims, demands, actions, or causes of action arising out of or related to any loss, injury, or damage that may be sustained by my child or my child's property during their participation in the event/activity. I also grant permission to Tiger Cho's Taekwondo to seek medical attention on my behalf if necessary, and I accept full responsibility for any medical expenses incurred. I have read and understood this Liability Waiver, and I am aware that by agreeing to my child's participation in the Tiger Cho's Taekwondo Afterschool Enrichment Program at Hickory Creek Elementary, I am voluntarily waiving certain legal rights, including the right to sue or seek compensation for any injuries or damages. I attest that my child is physically fit and has no medical conditions that would prevent their participation in the Tiger Cho's Taekwondo Afterschool Enrichment Program.*
