Waiver and Release of Liability
By submitting this form, I acknowledge that participation in the acting reel services provided by Powellful Productions may involve physical activity and/or the use of equipment. I voluntarily assume all risks associated with participation, including but not limited to personal injury, property damage, or other unforeseen incidents that may occur on or off set.
I hereby release and discharge Powellful Productions, its owners, affiliates, instructors, employees, contractors, and agents from any and all liability, claims, demands, or causes of action arising from participation in these services, whether caused by negligence or otherwise, to the fullest extent permitted by law.
Furthermore, I agree to adhere to all safety protocols and guidelines provided by Powellful Productions. I understand that failure to comply with these guidelines may result in dismissal from the activity without refund.
This release applies to all claims, whether known or unknown, anticipated or unanticipated. By submitting this form, I confirm that I have read, understood, and agree to these terms.
If the participant is a minor, a parent or guardian must acknowledge and accept these terms on their behalf.
