Hunter Registration & Biography

San Angelo Support for Veterans
Please fill out the form below to apply for consideration as a hunter on a SASFV sponsored hunt. We will check the accuracy of all information provided so please be truthful. Not all applicants will be selected in a given year, but we will keep your application on file for future hunts. If you are selected we will notify you via phone or email.

You will need to upload the following items with your application:
- Photos of Yourself - Current Photo & a photo of you in uniform.
- DD-214/s & DD-215/s - scanned documents with personal information marked out. Make sure that your name is still visible. Please provide all that you may have. We use these to help prevent fraud.
- Doctor Release Form (Active Duty Only)
Please note that the application consists of two parts.

The first section (Hunter Application) will remain confidential and will only be shared with SASFV staff for consideration to participate as a hunter.

The second section (Personal Bio) will be made public and and will be shared with ranch owners as well as the local newspaper for an article about the event which will contain bios about each hunter. Because of this, please note that there may be repeat fields in both sections but please only provide what you feel comfortable sharing in the public section.

Application is open to any current or former member of any branch of the United States Armed Forces who was injured or developed PTSD while in the line of duty.
