Please fill out this form clearly with all of the requested information and photos to ensure a smooth and efficient booking process. This will help prevent unnecessary back-and-forth and ultimately allow you to book in for your preferred date(s) much faster.

Please do not send duplicate forms as it may take longer to get back to you, and may result in miscommunication.

Christmas holiday closure
I will be on holidays from Fri 20th December until Mon 6th January. All emails and enquiry forms will be responded to on Monday 6th January. If you haven't received a response by 5pm on Monday 6th January please check your junk/spam folder before sending a follow up.

You must be as clear as possible about your design preferences to prevent unnecessary back-and-forth emails and to assist me in the design process. Please include any specific text (ie. names, dates, words, quotes), flower type preferences, colour preferences, etc.

Location photos

Please use the following fields to upload location photos for the design process. Adequate location photos allow me to design according to your specific body shape and size, reducing the need to redesign on the day.

Location photo requirements:
- best taken by someone else
- taken in natural indirect sunlight or under clear lighting
- taken with you standing in a relaxed and natural position
- photos must be parallel (flat) to each area, not angled in any way
For pieces not taking up the full area, it may be helpful to send a duplicate copy marked with the highest/lowest/widest points to include.

If you do not have someone able to take photos for you then we may organise a specific day/time for me to take photos in the shop, please request this in the 'other questions' box at the bottom of this form.

Example of the images required for a full sleeve:
- 1 x outer arm (shoulder cap to wrist) - stand side on to the camera, arm relaxed by your side
- 1 x inner arm (inner bicep to wrist) - stand facing the camera, raise your arm as if performing a lateral raise, turn your palm to face the camera
and a full leg:
- 1 x outer leg (highest point to ankle) - stand side on to the camera
- 1 x front leg (highest point to ankle) - stand facing the camera
- 1 x inner leg (highest point to ankle) - stand side on to the camera, stack your knee over your ankle, bring your front leg back so it does not obstruct the view
- 1 x back leg (highest point to ankle) - stand facing away from the camera
